0510674892 2018-09-25 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Identified myself to Company men Chuck Davies and Delbert Brown. Stage 55/57 is the next stage to be stimulated. Wireline set plug at 9902. Bottom perforation will be at 9889 and top perforation at 9749. Surface annular pressure was 0 and intermediate pressure was 132.
0652655780 2018-08-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 has moved off of location. A crew is onsite pressure washing the mats and stacking them for removal. Pad drains are shut. Containment is still in place. Seep on outslop of pad mentioned in previous ispection was viewed today. Approximately 40 sq ft of reseeded area is still not growing and the soil is moist. Made a small depression with my boot to test pH. pH was 12. Pictures were sent to Hydrologist Scott Scheiner and Engineer Paul Logan.
0722581931 2018-09-12 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Identified myself to company man Gilbert Agular and John Talbot. This well is next to stimulate. Stage 25/57 was perforated from 15059-15199 with plug set at 15212. They plan on pumping 1000 gallons of 7.5% HCl and 175000 lbs of sand, 40000 of 100 mesh and 135000 lbs of 40/70. Surface annular pressure is 0 and intermediate is 450. 5 guns with 6 shots on 35 foot spacing used to perforate.
0776540292 2018-11-05 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad containing 3 producing wells. Identified myself to company man Shaun Gallager and lease operator Shaun Garrettson. This well is producing off of natural reservoir pressure through the 5.5 inch diameter casing. Surface annular pressure is 2 and intermediate annular is 39. Production casing pressure is 4892. Production equipment includes 3 line heaters, 3 GPUs, 6 sand seperators, and 2 500 barrel steel produced water tanks in their own containment. Wells are producing from both sides of wellhead.Site meets requirements of OAC 1509.072A for preliminary restoration.
0853953842 2019-06-07 Preliminary Restoration No Violations On the day of my inspection I witnessed this pad to contain three producing wells. Well is producing through production casing and is flowing from both sides of the wellhead. Well is producing off of natural reservoir pressure. Production equipment includes 3 line heaters, 3 GPUs, 6 sand seperators, 2 500 barrel steel tanks in their own containment, and 1 combustor. The 13.375x9.625 annular pressure is zero and the 9.625x5.5 annular pressure is 600 psi. Site is stable and vegetated. Pad meets requirements of 1509.072(A) for preliminary restoration.
0932094040 2018-10-02 Completion Testing No Violations On the day of my inspection I witnessed the Halliburton frac crew has moved off of location. Three wells are on location. Surface annular pressure is 0 and intermediate annular is 0. Completions rig is expected on location late today. Returned to this pad later in the day to collect a sample (DEP201) of water seeping from the outslope of the pad. Amanda Finn and Greg McCutheon, both of Ascent, were on location briefly while I was filtering the water sample. Sample was delivered to New Philadelphia office and transferred to Kyle Tharp. Ascent has submitted a plan to fix this seep and DOGRM Engineers have approved their plan. Construction is slated to start 10/5.
1143765803 2018-06-20 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 drilling their 12.25 inch diameter hole at 7200 feet. Hole is at a 10.56 degree tangent. 9.625 inch diameter cement job estimated for Friday. Walk around location revealed no issues with containment. They are currently drilling on air.
1183859397 2018-05-09 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Falcon 26 drilled a 17.5 inch diameter open hole to a depth of 1410 feet. Previous casing set and cemented was 30 inch diameter conductor casing to a depth of 100 feet. Wyoming casing ran 1401 feet of 13.375 inch diameter casing, j-55, 61 lbs/foot. Casing was centralized with 12 centralizers. Shoe track is 43 feet long. CJ/O-tex was onsite with 1420 sacks of class A cement. Additives include 1.5% CaCl, 0.2% X-air, and .25 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.2. 50% excess ran in open hole. Cement displaced with 206 barrels of fresh water. 35 barrels of gel pumped ahead of cement. 66 barrels of cement returned to surface, and floats held with 1 barrel back.
1434770992 2018-06-28 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Pioneer 77 drilling their lateral section with an 8.5 inch diameter bit at a depth of 11540 feet. Kick off point for curve is 8455 and landing point was 9700 feet. There is a small flare burning and they are drilling with 14.3 ppg oil based mud. Planned total depth is 19554 feet.
1524754985 2018-04-26 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that Express Energy Services has 2 auger rigs on location. Both rigs are drilling. The cellar is set and 36 inch diameter hole has been augered to 82 feet. Driller believes they will reach total depth of 100 feet this afternoon and has casing on the way. Company man Doug Bennett plans to set up cement for tomorrow or Saturday.

Stage 43 plug will be set at 12026, bottom perforation at 12013, and top perforation at 11908. Surface annular pressure was 0 and intermediate was 138.
