0035321754 2017-09-21 Completion Testing No Violations On the date of my inspection I found a BJ Services crew rigged up on all 3 wells on the pad for hydraulic fracturing operations. While I was on site I witness BJ Services pump the frac job for stage 11 of 54 on this well and stage 11 of 56 on permit 3408120723. They are currently on stage 10 of 58 on this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0550786994 2017-05-10 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1520 feet and set 1507 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1600 sacks of class H cement, 1.19 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 85 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1180 psi for 5 minutes.
1493614839 2017-04-27 Drill / Deepen / Reopen No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and attempted to run 100 feet of 30 inch diameter 118 pound per foot conductor casing. While running casing, they could not get past the second joint. Precision had to ream the hole out with a 42 inch auger down to 60 feet to get past the blockage then ream the rest of the hole out. Once the hole is cleaned out they will attempt to run the casing again.
1493648933 2017-04-28 Casing String Witnessed No Violations Upon arrival on location I found the crew still drilling the hole for the conductor pipe from the Dickson 3H. The cement top for the 1H was tagged 1 foot below ground level. The Bates cement was tagged at 67 feet. 400 sacks of class A cement was pumped with no returns to surface. We shut down for 1 hour and then another 200 sacks of class A cement was pumped with no returns. Shut down and waiting on more cement.

1608825186 2017-08-22 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 292 rigging down. The rig is currently being hauled to another location by Warren Trucking. The dirty rig pieces are being cleaned with pressure washers within containment. The fluid produced by the cleaning process is being hauled off site for disposal. The pad drains are in the closed position and do not appear to be leaking.

1776450808 2017-08-02 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 292 drilling the intermediate section of this well at approximately 3000 feet. During my inspection I found the plastic containment berm on the high traffic side of the rig to be completely flattened. The containment is not capable of holding water if the berms are compressed. I spoke with the Ascent company man about the issue and he indicated that he would call the containment contractor out to repair the damaged areas as soon as possible. All of the chemicals were within containment. Areas of the pad appear to have subsided while the rig has been on site. Most of the water from the rig containment drains to the northwest corner of the pad containment. Ascent is having issues keeping up with the water when it rains. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
2082791881 2017-08-16 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 292 drilling the production section of this well at approximately 19000 feet. Ascent expects to reach a total depth of approximately 20800 feet by 8/17/17. During my inspection I found the containment near the casing rack had been torn open and was allowing any fluid near the front of the rig to drain out onto the pad. I spoke with the Ascent company man about the issue and he indicated that he would call the containment contractor out to repair the damaged areas as soon as possible. All of the chemicals were within containment. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
