0012713258 2017-06-08 Casing String Witnessed No Violations I am onsite to witness the topping out of the conductor annulus with cement. When I arrived I verified that the cement had fallen to approximately 3 feet below the bottom of the cellar. Smith Concrete topped the annulus out with grout mix. The floor of the cellar was brought to approximately 7 feet below ground surface, leaving an 18 inch floor of grout in the cellar.

1008832559 2017-06-06 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 320 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 74 barrels of water, leaving 10 feet of cement in the casing. Cement did returns to surface then fell back. During displacement water returned to surface in permit 3408120720, showing that the boreholes were communicating. Both wells will be topped out on 6/7/17 using grout pumped through a 1 inch line.

2019902726 2017-06-14 Drill / Deepen / Reopen No Violations I spoke with Steve Perkins regarding complaint number 5624. Ascent did a preliminary evaluation of the situation on 6/13/17. Ascent plans to have the site engineer Rettew re-evaluate the drainage for the lower portion of the lease road to determine how best to alleviate the excess water issue. They also determined that the mold growth within the structure was likely due to other issues. Ascent indicated that they should have the drainage re-evaluation complete by 6/23, and would keep me aware of their plan.
