0367011188 2017-05-22 Casing String Witnessed No Violations Upon arrival on location I found the crew had drilled a 36 inch diameter hole and installed 30 inch diameter casing to a depth of 100 feet. Circulation was established and 76 bbl. of cement was displaced with 70 bbl. of fresh water leaving around 10 feet of cement inside of the casing. 37 bbl. of good cement was circulated to surface.
0641716682 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found pad to contain 3 wells that arent yet labeled. Two wells, this one and permit 20710, have been drilled to total depth while the third well, 20709, appears to only have the conductor set. No pressures are shown any of the gauges on wellhead. Production equipment includes two line heaters, four sand seperators, two gas production units, and two 500 barrel steel tanks. Wells are not in production. There are also 2 empty frac tanks location. There is pipeline activity in the vicinity of the pad. Pad is vegetated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration.
1206445770 2017-06-19 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1465 feet and set 1554 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 35 barrels of spacer with 10 barrels of water behind before starting cement. They then pumped 1620 sacks of class A cement, 1.19 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 80 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1250 psi for 5 minutes.
1451434271 2017-08-22 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 292 moving onto the location. Currently only the living quarters, frac tanks, and drill pipe have been staged on the pad. The pad is surrounded by healthy vegetation and the pad drains are in the closed position. The rig will be rigging up on this well first. The surface casing for this permit has already been set.
1497800845 2017-06-18 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1465 feet and set 1554 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 35 barrels of spacer with 10 barrels of water behind before starting cement. They pumped approximately half of the cement and the bulk supply line became plugged. It took approximately 20 minutes to re-establish the connection. The Ascent superintendent decided at that point to circulate the cement out of the hole.
1509468855 2017-10-31 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on both wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 7 of 60 for this well and perforate stage 7 of 54 for permit 34-081-2-0710. During the frac, Halliburton pumped approximately 349000 pounds of sand using approximately 6200 barrels of water. They were able to achieve pumping rates of 80.7 barrels per minute during the job. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
1762140898 2017-11-20 Completion Testing No Violations On the date of my inspection I found a Halliburton Frac crew rigging down after successfully completing 60 frac stages on this well. They estimate that drill out operations should begin on the pad within 2 weeks.
1816796998 2017-08-31 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 292 drilling the intermediate section of this well at approximately 7800 feet. The Ascent company man believes they will reach their total depth for this string on 9/1/17. All of the chemicals onsite are within containment. The containment on the pad appears to be free of any visual punctures or tears. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
