0063069219 2017-09-14 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson rig 577 drilling the production section of this well at approximately 16440 feet. During my inspection I found a tear in the containment near the process water load out area. The containment in this area is also full and currently overflowing onto the pad. All of the chemicals were within containment with the exception of 2 pallets that are compressing the containment berm. I notified the company man of the issues I found and he indicated that he would take care of them. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
1194905692 2017-06-28 Drill / Deepen / Reopen No Violations On the date of my inspection I found a lease road with a locked gate. The pad has healthy vegetation growing around it and on the fenced berm. The pad has 3 set conductors. I verified that the cement has been brought to a depth of approximately 6 feet, leaving a 18 to 20 inch cement barrier between the ground and the inside of the cellar.
1279297031 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found pad to contain 3 wells that arent yet labeled. Two wells, this one and permit 20706, have been drilled to total depth while the third well, 20707, appears to only have the conductor set. No pressures are shown any of the gauges on wellhead. Production equipment includes two line heaters, four sand seperators, two gas production units, and two 500 barrel steel tanks. Wells are not in production. There are also 6 empty frac tanks location. Pad is vegetated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration.
1502173552 2017-08-08 Drill / Deepen / Reopen No Violations Today I am on site to witness the second hydrostatic test on the conductor cement job. Falcon Drilling rig 26 drilled to 140 feet, 10 feet below the last total depth, and loaded the hole with water. I witnessed the water level remain static in the pipe for 30 minutes.
1502368688 2017-08-09 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1465 feet and set 1453 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1630 sacks of class A cement, 1.19 yield and 15.6 pound per gallon, 1 percent calcium chloride, 0.25 pound cello flake per sack, 5.22 gallons per sack water requirement. Approximately 90 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1800 psi for 5 minutes.
