0019036126 2018-03-14 Drill / Deepen / Reopen No Violations On the day of my inspection I found pad to contain 3 wells that arent yet labeled. Permit 20706 and 20708, have been drilled to total depth while the third, this well, appears to only have the conductor set. Production equipment includes two line heaters, four sand seperators, two gas production units, and two 500 barrel steel tanks. Wells are not in production. There are also 6 empty frac tanks location. Pad is vegetated and stable. Per ORC 1509.072A this permit is not eligible for a preliminary restoration until it is drilled to total depth
0600444291 2017-06-19 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 130 feet and ran 130 feet of 30 inch diameter 118 pound per foot conductor casing. A mine void was encountered from approximately 83 feet to 91 feet below the surface. A cement basket was set at approximately 70 feet. The basket was placed approximately 5 feet above a coal seam that was encountered while drilling above the mine void. O-Tex pumped 280 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 1.5 percent Calcium Chloride and 0.25 pound per sack flow seal followed by 2 barrels of water to balance the job. The basket was opened using approximately 200 pounds of gravel and 3 bags of calcium chloride. O-Tex then ran one 10 foot length of 2 inch line down beside the conductor and pumped 40 barrels of cement until it came to surface. Cement returned to surface and remained static. Cement was brought to approximately 7 feet below ground surface, leaving a approximately 18 inches of good cement as a floor in the cellar.
