0271031759 2017-03-15 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Rocky Mountain Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 400 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal. I witnessed good cement returns to surface and remain static.
0394920566 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 16 psi surface, 220 psi intermediate, 4200 psi tubing, and 3000 production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
0627776471 2017-04-19 Casing String Witnessed No Violations Purpose of this inspection is a status check. Onsite to witness a hydrostatic test on the 18 5/8 inch diameter mine string casing. Casing set to 316 feet and open hole drilled through shoe to 332 feet. Fluid level was viewed for a half hour and dropped approximately 18 inches in the 30 minutes.
0825658156 2017-09-20 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad has 3 wells that have been drilled. The wellheads are surrounded by cement barriers. Currently there are 7 frac tanks and the walls for an above ground storage tank being stored onsite. Ascent plans to begin frac operations on these wells within the next month. The hillside on the east side of the pad where the ground bad been scorched by a large flare during the drilling process has healthy vegetation now growing on it. The chain link fence on the eastern side of the pad has partially bent over towards the pad. This occurred while the drilling rig was onsite due fastening containment onto the fence to prevent overspray from leaving the pad.
0993659487 2017-11-07 Completion Testing No Violations On the date of my inspection I found a BJ Services crew rigged up on all 3 wells on the pad for hydraulic fracturing operations. While I was on site I witness BJ Services pump the frac job for stage 33 of 56 on this well. During the stage they pumped approximately 250000 pounds of sand using approximately 6248 barrels of water at rates as high as 86 barrels per minute. They are currently on stage 32 of 49 on permit 34-081-2-0702, and stage 33 of 56 on permit 34-081-2-0704. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
1061658991 2017-10-16 Completion Testing No Violations On the date of my inspection I found a BJ Services crew rigging up on all three wells on this pad in preparation for hydraulic fracturing operations. They are currently rigging up the frac stacks on the wells and moving in pump trucks. Allied Horizontal wireline services it the wireline contractor. Ascent anticipates operations to begin on 10/18/17 if setup continues to go smoothly. The pad is surrounded by a fenced earthen berm that is covered with healthy vegetation. The pad drain valves are in the closed position. The hillside that had been scorched by a large flare during the drilling process has healthy vegetation growing on it again.
1304285105 2017-07-11 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 714 drilling the out the DV tool from the 9.625 inch intermediate casing cement job. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The hillside near the flare stack that had been scorched and misted with oil has been remediated. Ascent removed the top layer of soil and seeded the area. All of the chemicals are being stored within containment. The portion of containment behind the rig has been compressed to the point that it will not hold water in several places. I notified the company man who said he would have a crew called in to repair the plastic containment berm by the end of tomorrow. The pad drain valves are in the closed position and do not appear to be leaking.
1492495632 2017-04-18 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Falcon Drilling rig 26 drilled a 24 inch diameter hole to a depth of 321 feet and set 318 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 660 sacks of class H cement, 1.2 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. A coal seam was identified from 262 feet to 272 feet and two baskets were set at 253 feet and 250 feet. Cement did not circulate to surface. O-Tex pumped 6 barrels of class H, 1.2 yield, 15.6 pound per gallon cement mixed with calcium chloride and baro-seal through approximately 240 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket. Approximately 86 barrels of cement was pumped between 4 top out attempts before it returned to surface and remained static. O-Tex waited approximately an hour between each attempt.
1492710856 2017-04-20 Casing String Witnessed No Violations Falcon 26 drilled a 17.5 inch open hole to a depth of 1720 feet. Wyoming Casing set 1706 feet of 13.375 inch 61# casing through 316 feet of 18.675 #87.5 mine string. 13 centralizers were ran. OTEX onsite to cement with 1470 sks of class H cement with a yield of 1.2 and weight of 15.6. Displacement of 246.6 bbl water will leave an 85.25 foot shoe. 50% excess cement is calculated for open hole. Hole was loaded prior to starting job and 80 bbl cement returns to surface. Floats held.
1737648116 2017-07-27 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 714 running the cement bond log on the 9.625 inch intermediate casing after drilling the well to total depth. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The hillside near the flare stack that had been reseeded is now covered with vegetation. All of the chemicals are being stored within containment. The portion of containment behind the rig has been compressed from being driven over repeatedly. There is a tear in the containment near the air compressors. I notified the company man who said he would have the containment repaired and a metal cross over placed on the compressed area to prevent it from happening again. The pad drain valves are in the closed position and do not appear to be leaking.
