0359333168 2017-10-16 Completion Testing No Violations On the date of my inspection I found a BJ Services crew rigging up on all three wells on this pad in preparation for hydraulic fracturing operations. They are currently rigging up the frac stacks on the wells and moving in pump trucks. Allied Horizontal wireline services it the wireline contractor. Ascent anticipates operations to begin on 10/18/17 if setup continues to go smoothly. The pad is surrounded by a fenced earthen berm that is covered with healthy vegetation. The pad drain valves are in the closed position. The hillside that had been scorched by a large flare during the drilling process has healthy vegetation growing on it again.
0362892284 2017-04-22 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Falcon Drilling rig 26 drilled a 24 inch diameter hole to a depth of 320 feet and set 312 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 660 sacks of class H cement, 1.2 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. A coal seam was identified from 262 feet to 272 feet and two baskets were set at 248 feet and 246 feet. Cement did not circulate to surface. O-Tex pumped 5 barrels of class H, 1.2 yield, 15.6 pound per gallon cement mixed with calcium chloride and baro-seal through approximately 240 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket. O-Tex waited an hour and then 400 sacks of cement. Approximately 10 barrels of good cement returned to surface.
1251583467 2017-04-23 Casing String Witnessed No Violations I am onsite today to witness the hydrostatic test for the 18.625 inch diameter mine string. Falcon Drilling rig 26 drilled to 335 feet, 15 feet beyond the last total depth of 320 feet. Fresh water was pumped into the 18.625 inch casing and brought to the top where it remained static for 30 minutes.

1489441469 2017-03-13 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Rocky Mountain Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 400 sacks of class A cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal. I witnessed approximately 25 barrels of good cement return to surface. A vac truck was used to remove the excess cement, leaving approximately 18 inches of good cement in the cellar.

1493125527 2017-04-25 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1720 feet and set 1713 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1470 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 75 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1265 psi for 5 minutes.
1496322879 2017-06-01 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 359 is currently moving onto the pad and rigging up on the 5H. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. Chesapeake plans to spud the intermediate string on 6/3/17 if the rig moves goes according to plan.
1499742825 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 130 psi surface, 500 psi intermediate, 4700 psi tubing, and 3800 production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
1507214120 2017-06-15 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 359 is currently drilling the curve portion of the production string at approximately 10030 feet. The pad is in good condition. The vegetation on the inside of the berm near the half-round is turning brown while the vegetation on either side is healthy. The chain link fence on the top of the berm near the half-round is red from the over spray created while drilling the intermediate portion of the well. The vegetation on the outside of the fence near the half-round area looks stressed and is beginning to turn brown. The pad drain valves are closed and do not appear to have any leakage issues. Containment is visibly free of tears or punctures. Multiple pallets and drums of chemicals are not within the primary containment. I notified that Ascent company man and the superintendent about the containment and overspray issues. The superintendent indicated that both issues would be remedied. I will return in a few days to check on the vegetation outside of the fence to determine if any pollution of the environment has occurred. Pictures were taken.
1548723967 2017-09-20 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad has 3 wells that have been drilled. The wellheads are surrounded by cement barriers. Currently there are 7 frac tanks and the walls for an above ground storage tank being stored onsite. Ascent plans to begin frac operations on these wells within the next month. The hillside on the east side of the pad where the ground bad been scorched by a large flare during the drilling process has healthy vegetation now growing on it. The chain link fence on the eastern side of the pad has partially bent over towards the pad. This occurred while the drilling rig was onsite due fastening containment onto the fence to prevent overspray from leaving the pad.
1686876538 2017-06-21 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 359 is currently drilling the production string at approximately 14793 feet. The pad is in good condition. The vegetation on the inside of the berm near the half-round does not look as healthy as the surrounding grass. The containment has been extended up the fencing to help with mist migrating off site once they start drilling the intermediate portion of the next well. I spoke with Ascent company man Buzz about this and he indicated that they are working on a better solution to contain the half-round more completely. The grass area outside the fence adjacent to the flare is all brown. The company man indicated that they had a large sustained flare that scorched the grass. The pad drain valves are closed and do not appear to have any leakage issues. The company man indicated that they are hauling all of their drainage water off site for disposal due to excessive chloride content. All chemicals are now within containment. However, the containment has large holes in it. The company man indicated that he would be re-arranging the chemical storage for this location due to the small size of it but would have the tears repaired today.
