0063590695 2017-12-11 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad has 5 recently stimulated wells, 10 sand separators, 5 gas production units, and 2 shared 500 barrel tanks within a metal dike. Identification has not been placed on the wellheads yet. The wellheads are equipped with christmas tree valves. There are 2 flowlines coming from each wellhead. Currently Express Energy Services and ROC Service Company are onsite rigging up flowback equipment on the 5H, 7H, and 8H. Ascent anticipates flowback operation to begin on 12/12/17.
0402166855 2017-09-15 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on all three wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 28 of 62 on permit 3408120698. They are currently on stage 10 on this well and had to stop operations on it to swap out wellheads. They decided continue frac operations on this well once they rig up on the last 2 wells on the pad. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0649647543 2017-10-02 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on all three wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 25 of 70 for this well and stage 11 of 56 for permit 34-081-2-0669. During the frac, Halliburton pumped approximately 331000 pounds of sand using 6885 barrels of water. They were able to achieve an average pumping rate of 68 barrels per minute during the job. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0906573762 2017-08-16 Completion Testing No Violations On the date of my inspection I found 9 Energy Services running a cement bond log on permit 34-081-2-0699 with a wireline truck. Great Lake Wellhead Inc. was also onsite installing wellheads. Currently there are 5 frac tanks and a few miscellaneous pieces of equipment onsite that are associated with the operations currently being performed on the pad. The pad berms and the area immediately adjacent to the pad have been recently hydroseeded.
0909522041 2017-03-12 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1430 feet and set 1415 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1380 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 33 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1150 psi for 5 minutes.
2010003469 2018-04-16 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 5 properly labeled wells that are producing. This well is equipped with christmas tree style wellhead that is much taller than normal. Speaking with Lease Operator, he said that there is no tubing is the well and it is produced through the production casing. He also said that he put in a work order to have the intermeidate annualar blown down. Pressure gauges read as follows: Surface 14 psi, Intermediate 2024 psi, Production 3473 psi. Production equipment includes two 500 barrel steel tanks within their own steel containment, five line heaters, and five GPUs. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
2055124044 2017-03-03 Casing String Witnessed No Violations I am on site today to witness the cementing of 24 inch conductor casing. Precision Drilling drilled a 30 inch diameter hole to a depth of 125 feet and ran 130 feet of 24 inch diameter 96 pound per foot conductor casing. Allied pumped 407 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal. I witnessed good cement returns to surface and remain static.
2077483414 2017-10-30 Completion Testing No Violations On the date of my inspection I found the pad being prepared for drill out operations. The frac crew has moved off the pad after completing 70 stages on this well. Ascent estimates the drill out rig will be moving onto the pad within 8 days. Pressure gauges on the surface and intermediate casings read 0 and 2100 pounds per square inch respectively. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
