0190491014 2017-12-11 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad has 5 recently stimulated wells, 10 sand separators, 5 gas production units, and 2 shared 500 barrel tanks within a metal dike. Identification has not been placed on the wellheads yet. The wellheads are equipped with christmas tree valves. There are 2 flowlines coming from each wellhead. Currently Express Energy Services and ROC Service Company are onsite rigging up flowback equipment on the 5H, 7H, and 8H. Ascent anticipates flowback operation to begin on 12/12/17.
0536954984 2017-10-02 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on all three wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 25 of 70 for permit 34-081-2-0700 and stage 11 of 56 for permit 34-081-2-0699. Halliburton has stimulated stage 10 of 53 states on this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0845190910 2017-03-06 Casing String Witnessed No Violations I am on site today to witness the cementing of 24 inch conductor casing. Precision Drilling drilled a 30 inch diameter hole to a depth of 125 feet and ran 125 feet of 24 inch diameter 96 pound per foot conductor casing. A 10 foot mine void was found from 70 to 80 feet, a basket was ran at approximately 55 feet. O-Tex pumped 220 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 20 barrels of water to set the shoe. 1 bag of calcium chloride along with 1 bag of cotton seed hulls were poured down the backside of the 24 inch casing into the basket. O-Tex ran 10 feet of hose down the hole on one side of the casing to pump cement and ran another hose down the other side to pour approximately 10 gallons of Sodium Silicate to flash set the cement in the basket. O-Tex pumped approximately 122 sacks of cement until it returned to within 10 feet of surface and remained static. Ascent will come back and top out with ready-mix.
1222926049 2017-08-16 Completion Testing No Violations On the date of my inspection I found 9 Energy Services running a cement bond log on permit 34-081-2-0699 with a wireline truck. Great Lake Wellhead Inc. was also onsite installing wellheads. Currently there are 5 frac tanks and a few miscellaneous pieces of equipment onsite that are associated with the operations currently being performed on the pad. The pad berms and the area immediately adjacent to the pad have been recently hydroseeded.
1316637386 2017-03-08 Casing String Witnessed No Violations Today I verified that the remaining annulus and cellar were topped out with ready-mix. Ascent has filled the floor of the cellar with ready-mix, leaving an approximately 18 inch barrier between the cellar floor and soil below. Ascent plans to install the mouse hole after a top hole rig completes the surface string, before a big rig is brought in to complete the remaining strings.
1317630211 2018-04-16 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 5 properly labeled wells that are producing. This well is equipped with christmas tree style wellhead. Pressure gauges read as follows: Surface 10 psi, Intermediate 1625 psi, Production 3100 psi, and tubing was 1400 psi. Production equipment includes two 500 barrel steel tanks within their own steel containment, five line heaters, and five GPUs. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
1339499074 2017-05-30 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Patterson drilling rig 284 is currently drilling the intermediate string at approximately 7400 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. The containment areas appear to be free of tears and punctures. All hazardous materials are within containment areas.
1490091842 2017-03-21 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1433 feet and set 1420 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1420 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 70 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1170 psi for 5 minutes.
1497816229 2017-10-30 Completion Testing No Violations On the date of my inspection I found the pad being prepared for drill out operations. The frac crew has moved off the pad after completing 63 stages on this well. Ascent estimates the drill out rig will be moving onto the pad within 8 days. Pressure gauges on the surface and intermediate casings read 0 and 700 pounds per square inch respectively. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
