0180225701 2017-03-06 Casing String Witnessed No Violations I am on site today to witness the cementing of 24 inch conductor casing. Precision Drilling drilled a 30 inch diameter hole to a depth of 125 feet and ran 125 feet of 24 inch diameter 96 pound per foot conductor casing. A 10 foot mine void was found from 70 to 80 feet, a basket was ran at approximately 55 feet. O-Tex pumped 220 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 20 barrels of water to set the shoe. 1 bag of calcium chloride along with 1 bag of cotton seed hulls were poured down the backside of the 24 inch casing into the basket. O-Tex ran 10 feet of hose down the hole on one side of the casing to pump cement and ran another hose down the other side to pour approximately 10 gallons of Sodium Silicate to flash set the cement in the basket. O-Tex pumped approximately 122 sacks of cement until it returned to within 10 feet of surface and remained static. Ascent will come back and top out with ready-mix.
0200345346 2017-10-30 Completion Testing No Violations On the date of my inspection I found the pad being prepared for drill out operations. The frac crew has moved off the pad after completing 63 stages on this well. Ascent estimates the drill out rig will be moving onto the pad within 8 days. Pressure gauges on the surface and intermediate casings read 150 and 600 pounds per square inch respectively. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
0982926934 2017-08-16 Completion Testing No Violations On the date of my inspection I found 9 Energy Services running a cement bond log on permit 34-081-2-0699 with a wireline truck. Great Lake Wellhead Inc. was also onsite installing wellheads. Currently there are 5 frac tanks and a few miscellaneous pieces of equipment onsite that are associated with the operations currently being performed on the pad. The pad berms and the area immediately adjacent to the pad have been recently hydroseeded.

1488983181 2017-03-08 Casing String Witnessed No Violations Today I verified that the remaining annulus and cellar were topped out with ready-mix. Ascent has filled the floor of the cellar with ready-mix, leaving an approximately 18 inch barrier between the cellar floor and soil below. Ascent plans to install the mouse hole after a top hole rig completes the surface string, before a big rig is brought in to complete the remaining strings.
1489589190 2017-03-15 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1439 feet and set 1426 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1410 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 40 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1200 psi for 5 minutes.
1513016573 2017-12-11 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad has 5 recently stimulated wells, 10 sand separators, 5 gas production units, and 2 shared 500 barrel tanks within a metal dike. Identification has not been placed on the wellheads yet. The wellheads are equipped with christmas tree valves. There are 2 flowlines coming from each wellhead. Currently Express Energy Services and ROC Service Company are onsite rigging up flowback equipment on the 5H, 7H, and 8H. Ascent anticipates flowback operation to begin on 12/12/17.
1522250429 2017-03-13 Casing String Witnessed No Violations The purpose of my inspection was to witness the hydrostatic test of the 24 conductor/ mine string. Falcon Drilling Rig # 26 was on location to drill out the casing seat set at 130 measured depth. Falcon drilled to a total measured depth of 155 per company representative. I arrived on location to find Falcon #26 drilling the last joint to the total measured depth for the test. The hole was circulated clean prior to tripping out the drill string. After removing the drill string the hole was loaded with fresh water by the gravity method from a vacuum water truck. The water level was brought above surface into the converter head attached to the conductor casing. The static water level was monitored for thirty (30) minutes and did not fall. I communicated to the company representative Randy Sheppard that the test passed and operations could proceed as planned prior to leaving location.
1523896967 2018-04-16 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 5 properly labeled wells that are producing. This well is equipped with christmas tree style wellhead that is much taller than normal. Speaking with Lease Operator, he said that there is no tubing is the well and it is produced through the production casing. Pressure gauges read as follows: Surface 120 psi, Intermediate 774 psi, Production 2025 psi. Production equipment includes two 500 barrel steel tanks within their own steel containment, five line heaters, and five GPUs. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
