0021754392 2017-08-16 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 577 tripping out of the hole after reaching their total depth for this section of 8657 feet. Ascent plans to run the 9.625 inch casing this afternoon and cement on 8/17. This is a follow up inspection from 8/14, when I found a brine saturated area on the pad. The leak has not been fixed yet. The water tested for between 1127 and 1222 parts per million using Quantabs on 8/14 and tested for 1694 parts per million today. I spoke with the Ascent company man about the leak and he called a containment company while I was there to come out to work on the issue.

0739239165 2017-12-13 Completion Testing No Violations On the date of my inspection I found a well maintained access road with a locked gate. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. There are 4 recently drilled wells on the pad surrounded by concrete barriers. The wells have not been hydraulically fractured yet. Containment for frac operations has been placed around the wellheads. There are currently 8 frac tanks, a trailer, a forklift, and walls for an above ground storage tank onsite. The two 500 barrel tanks for production have been installed and are within a metal dike.
0840471632 2018-02-23 Preliminary Restoration No Violations On the day of my inspection I witnessed Fractech was onsite under direction of Ascent Company man Larry Jennings. This pad is splitting water with the Giusto pad, and the other pad is currently receiving water for frac. Stage 1 is finished. Received call from Mr. Niece (ODOT aviation) on 2/21 in regards to crane not having a permit from ODOT. Pad is within 10000 feet of jefferson airpark and ODOT has permits for erections that close to airport. A temporary light was installed on crane. Permanent light is supposed to arrive on location today or tomorrow. Outslope of pad is vegetated and stable. Lease road is stable. Site meets requirements of ORC 1509.072A for preliminary restoration.

1496226847 2017-05-31 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1435 feet and set 1415 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1500 sacks of class A cement, 1.19 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 80 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1050 psi for 5 minutes.
1503409285 2017-08-22 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 577 performing the clean up cycle on the hole before coming out to run the 5.5 inch production casing. They reached their total depth of 17778 last night. Ascent plans to run casing on 8/23 and expects to cement the production casing on 8/24. There is standing water along the edges of the pad on top of the french drains due to heavy rains 3 days ago. Ascent is having trouble getting water trucks out to remove the water. They are unable to release the water due to excessive chlorides. The containment breach near the flare has been repaired. The pad is surrounded by healthy vegetation. The pad drains are in the closed position and do not appear to be leaking. Currently a pipeline crew is preparing the right of way adjacent to the pad.
1739062903 2017-08-14 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 577 drilling the intermediate section of this well at approximately 6200 feet. Ascent anticipates reaching their total depth for this string tomorrow and running casing on 8/16/17. During my inspection I found multiple punctures and tears in the containment near the catwalk. I found a saturated area on the pad between the Marybeth and the flare that appears to be leaking from within the containment. The water tested for between 1127 and 1222 parts per million using Quantabs. I spoke with the Ascent company man about the issues and he indicated that he would call the containment contractor out to repair the damaged areas today and would have them locate the leak that the brine is coming from. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pad drain valves are in the closed position and do not appear to be leaking.
1862895797 2017-04-13 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 400 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 75 barrels of water, leaving 10 feet of cement in the casing. I witnessed good cement return to surface and remain static
