0085650614 2017-08-10 Completion Testing No Violations On the date of my inspection I found a Keane Frac crew rigged up on both wells on the pad. While I was on site I witnessed Keane pump the frac job for stage 74 of 82 for the 4H well. This well has had 71 stages of 83 completed at the time I was on site. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position. Ascent said they should finish up the operations on the pad in 2 more days.
0419957693 2017-02-13 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. Express pumped 400 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal. I witnessed good cement returns to surface and remain static.
0803663633 2017-04-21 Casing String Witnessed No Violations On the date of my inspection I found Nomac 84 attempting to wash down the 18.625 inch casing. They drilled a 24 inch hole to 354 feet and intended to set 354 feet of casing. A void was identified from 295 to 310 feet. They have been trying to work the casing down for approximately 6 hours and are stuck 10 feet below the mine. Ascent will have Nomac 84 pull the casing and then ream out the hole to bottom before running the casing again.
0895564981 2017-04-23 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Nomac Drilling rig 84 drilled a 24 inch diameter hole to a depth of 354 feet and set 348 feet of 18.625 inch, 87.5 pound per foot casing. Schlumberger pumped 263 sacks of class H cement, 1.2 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. A void was identified from 295 feet to 310 feet and two baskets were set at 276 feet and 280 feet. Cement did not circulate to surface. Schlumberger pumped 10 barrels of class H, 1.2 yield, 15.6 pound per gallon cement mixed with calcium chloride and baro-seal through approximately 260 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket. Schlumberger waited an hour in between each 10 barrel top out. On the third top out cement returned to surface and remained static.
0896079584 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 2 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 0 psi surface, 500 psi intermediate, 800 psi tubing, and 2300 psi production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.

Nomac Drilling rig 84 drilled to 367 feet, 13 feet beyond the last total depth of 354 feet. Fresh water was pumped into the 18.625 inch casing and brought to the top where it remained static for 30 minutes.
1314098087 2017-05-15 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 84 is currently running the 5.5 inch production casing at approximately 19,000 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. Containment appears to be in good condition with no apparent holes or tears. All hazardous materials are within containment areas.

1493102237 2017-04-25 Casing String Witnessed No Violations Upon arrival, I found the rig crew had drilled a 17.5 diameter hole and installed 1714 feet of 13.375-inch diameter casing with a 44.55-foot shoe track and the float color set at 1667.50. Schlumberger will be doing the cementing with 1540 sacks of Class H 16.5 lb. cement. Circulation was established, circulated 35 bbl. of good cement returns to surface. Floats held and last sample weighed 16.2 lbs. job complete.
1502982536 2017-08-17 Completion Testing No Violations On the date of my inspection I found Deep Well Services drilling out the plugs set by the frac crew. They are currently at 16000 feet and expect operations on this well to take until 8/21 or 8/22. After they complete drill out operations on this well they will move to the 4H (permit 34-081-2-0691). All chemicals are within containment. The pad drains are in the closed position and do not appear to be leaking.

2127704339 2017-07-11 Completion Testing No Violations On the date of my inspection I found Keane Frac rigged up to begin hydraulic fracturing on both of the wells on this pad. I spoke with the Ascent company man who indicated that the operations should begin this afternoon and that they were waiting on their water supply. He indicated that they should be on this location for approximately 3 weeks. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position. The company man indicated that any water that accumulates within the containment is removed using a vac truck then transferred into the water holding tank on site and used for the fracturing process. The 13.375 inch casing gauge showed zero pounds per square inch while the 9.625 inch casing gauge showed 2100 pounds per square inch.
