0063759271 2017-08-25 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on all three wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 42 of 52 and Schlumberger perforate stage 43. During the frac job, Halliburton pumped approximately 228000 pounds of sand using approximately 4817 barrels of water. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0271164162 2016-12-06 Drill / Deepen / Reopen No Violations On the date of my inspection I found a newly completed well pad. The construction of the pad has just finished with the exception of installing a chain link fence. The gravel access road has a locked gate and the disturbed area around the pad including the earthen berm have been re-seeded and covered with straw.

0718869592 2017-08-04 Completion Testing No Violations On the date of my inspection I found Halliburton rigging up to begin hydraulic fracturing operations. They plan to zipper frac the wells (3) on this pad. Ascent estimates that they should begin frac operation by morning on 8/5/17. Ascent estimates that operations on the pad should take 3 to 4 weeks. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pipeline company is also preparing the outside of the pad for sales line installation. Traffic on the lease road is very heavy with both operations occurring at the same time.
0976571252 2017-05-31 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 77 is currently drilling the production string at approximately 10240 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. The containment areas appear to be free of tears and punctures. All hazardous materials are within containment areas.
0978140359 2018-03-08 Preliminary Restoration No Violations On the day of my inspection I found the Wilma pad to contain three properly identified producing wells. Wells have christmas tree style well heads. Annular pressures are: surface- 2, intermediate- 15, and production 1400. Production equipment includes six sand separators, three gas production units, three glycol dehydration units, and two 500 barrel steel produced water tanks within a metal dike. All production equipment is surrounded by concrete barriers. Site is vegatated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration.

1488457365 2017-03-02 Drill / Deepen / Reopen No Violations I am onsite today to witness the hydrostatic test for the 18.625 inch diameter mine string. I witnessed drilling the plug. Falcon Drilling rig 26 drilled to 360 feet, 10 feet beyond the last total depth of 350 feet. Fresh water was pumped into the 18.625 inch casing to surface and held static level for 30 minutes.
1822790749 2017-02-03 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. Universal Well Service pumped 400 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal. I witnessed good cement returns to surface. The returns fell back to approximately 1.5 feet. Ready mix was used to top out the annulus. Approximately 18 inches of good ready mix was left in the tin horn.

1888948298 2017-09-20 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad has 3 wells that have been drilled and stimulated and awaiting flowback, 4 sand separators, 3 gas production units, 3 glycol dehydration units, and 2 shared 500 barrel tanks within a metal dike. The well heads are equipped with christmas tree valves and surrounded by cement barriers. The pad is surrounded by an earthen dike covered by healthy vegetation. Currently SEI Oil Field Services is onsite installing the production equipment. The SEI foreman indicated that there will be a total of 6 sand separators, two for each well. There are 4 frac tanks as well as other equipment associated with operations on the pad currently on site.
1907650098 2017-03-01 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Falcon Drilling rig 26 drilled a 24 inch diameter hole to a depth of 350 feet and set 342 feet of 18.625 inch, 87.5 pound per foot casing. O-Tex pumped 700 sacks of class H cement, 1.2 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. A coal seam was identified from 286 feet to 297 feet and two baskets were set at 270 feet and 266 feet. No cement returned to surface. O-Tex pumped 6 barrels of class H, 1.2 yield, 15.6 pound per gallon cement through tremie pipe on the backside of the 18.625 inch casing to set the basket. After setting for 2 hours, O-Tex pumped 80 barrels of class H, 1.2 yield, 15.6 pound per gallon cement through tremie pipe on the backside of the 18.625 inch casing until cement returned to surface and remained static.
