0452220796 2017-08-15 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on all three wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 17 of 51 for permit 34-081-2-0664. Halliburton has completed stage 17 of 50 for this well. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
0571370665 2017-09-20 Completion Testing No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad has 3 wells that have been drilled and stimulated and awaiting flowback, 4 sand separators, 3 gas production units, 3 glycol dehydration units, and 2 shared 500 barrel tanks within a metal dike. The well heads are equipped with christmas tree valves and surrounded by cement barriers. The pad is surrounded by an earthen dike covered by healthy vegetation. Currently SEI Oil Field Services is onsite installing the production equipment. The SEI foreman indicated that there will be a total of 6 sand separators, two for each well. There are 4 frac tanks as well as other equipment associated with operations on the pad currently on site.

While I was on site I witnessed Halliburton pump the frac job for stage 42 of 52 and Schlumberger perforate stage 43 on permit 3408120666. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
1023105343 2017-08-17 Completion Testing No Violations On the date of my inspection I found a Halliburton crew rigged up on all three wells on the pad for hydraulic fracturing operations. While I was on site I witnessed Halliburton pump the frac job for stage 22 of 51 and Schlumberger perforate stage 23 for this well. During the frac, Halliburton pumped approximately 233300 pounds of sand using 4826 barrels of water. They were able to achieve pumping rates of up to 85.1 barrels per minute during the job. All hazardous materials are within double containment which is in good condition, free of visible punctures or tears. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position.
1486394274 2017-02-06 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. Allied pumped 400 sacks of class A cement, 1.18 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.75 pound per sack flow seal. I witnessed good cement returns to surface and remain static.
1488202423 2017-02-24 Casing String Witnessed No Violations I was onsite today to witness the cementing of the 18.625 inch diameter mine string. I was unable to witness the job. After pumping the first stage of the job, O-Tex did not circulate the cement out of the mix tub. When they went to start the top out job, they realized the cement had solidified in the mixing tank. O-Tex had another truck dispatched to the location.
1497532698 2017-06-15 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Nomac drilling rig 77 is currently drilling the lateral portion of the production string at approximately 10600 feet, they recently landed the curve at 10475 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. The containment around the catwalk has multiple tears. I notified that Ascent company man and he indicated that the containment repair crew were already schedule to fix it this afternoon. All hazardous materials are within containment areas.
1697968547 2017-08-04 Completion Testing No Violations On the date of my inspection I found Halliburton rigging up to begin hydraulic fracturing operations. They plan to zipper frac the wells (3) on this pad. Ascent estimates that they should begin frac operation by morning on 8/5/17. Ascent estimates that operations on the pad should take 3 to 4 weeks. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. The pipeline company is also preparing the outside of the pad for sales line installation. Traffic on the lease road is very heavy with both operations occurring at the same time.

1923284578 2018-03-08 Preliminary Restoration No Violations On the day of my inspection I found the Wilma pad to contain three properly identified producing wells. Wells have christmas tree style well heads. Annular pressures are: surface- 8, intermediate- 15, and production 1400. Production equipment includes six sand separators, three gas production units, three glycol dehydration units, and two 500 barrel steel produced water tanks within a metal dike. All production equipment is surrounded by concrete barriers. Site is vegatated and stable. Site meets requirements of ORC 1509.072A for preliminary restoration.
