0784309426 2017-04-13 Casing String Witnessed No Violations I am on site today to witness the cementing of 30 inch conductor casing. Precision Drilling drilled a 36 inch diameter hole to a depth of 100 feet and ran 100 feet of 30 inch diameter 118 pound per foot conductor casing. O-Tex pumped 400 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 75 barrels of water, leaving 10 feet of cement in the casing. I witnessed good cement return to surface and remain static
1003734642 2017-12-13 Completion Testing No Violations On the date of my inspection I found a well maintained access road with a locked gate. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. There are 4 recently drilled wells on the pad surrounded by concrete barriers. The wells have not been hydraulically fractured yet. Containment for frac operations has been placed around the wellheads. There are currently 8 frac tanks, a trailer, a forklift, and walls for an above ground storage tank onsite. The two 500 barrel tanks for production have been installed and are within a metal dike.

1495860878 2017-05-26 Casing String Witnessed No Violations On the day of my inspection I witnessed the 13.625 inch diameter surface cement job. Falcon 26 drilled a 17.5 inch diameter open hole to 1425 feet. Wyoming casing set 1410 feet of j-55, 61 lb. per foot, 13.625 inch diameter casing. Casing was centralized with 12 centralizers. Shoe track is 44.29 feet long. O-tex is onsite with 1510 sacks of class A cement with 1.19 yield, 2% CaCl, .2% X-air, and .25 lbs per sack celloflake. 50% excess cement ran in open hole. Cement displaced with 209 barrels of fresh water. 84 barrels of cement returned to surface, and floats held with 1.5 barrels back. Mix water had a pH of 7, 113 ppm chlorides, and 68 degrees.
1499951329 2017-07-13 Drill / Deepen / Reopen No Violations On the date of my inspection I found Patterson drilling rig 577 had just completed the production casing cement job. The total measured depth of the well is 20576 feet. Ascent plans to walk the rig over to the 1H this afternoon. The pad is surrounded by a fenced earthen dike covered by healthy vegetation. All of the chemicals are being stored within containment. The portion of containment around the catwalk has a few tears and a small area on the corner of the containment near the lease road entrance is also torn. I notified the company man who said he would have a crew called in to repair the plastic containment as soon as possible. The pad drain valves are in the closed position and do not appear to be leaking.
1567398217 2017-06-26 Drill / Deepen / Reopen No Violations I am on site today to perform a general site inspection. Currently Nomac drilling rig 77 is moving in and rigging up on this permit number. They intend to begin drilling the intermediate portion of the well on 6/27/17 if the move continues to go well. The pad appears to be in good shape with healthy vegetation on the berms.
