0137198058 2017-10-26 Drill / Deepen / Reopen No Violations On the day of my inspection Patterson 292 drilled 10 below the set mine string to a depth of 306. The hole was loaded with water and watched for 30 minutes. No fluid loss was noted. Hydrostatic test was successful and they can continue drilling for surface casing.
0215140361 2018-02-12 Work Over Reconditioning No Violations On the day of my inspection I met company man Dave Rigatoni. Fracking crew finished up in late January. Deep well rig # 4 drilled out plugs and snub #2 is running tubing back into the well.
0637945185 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 23 psi surface, 963 psi intermediate, 5851 psi tubing, and 3300 psi production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
1258183488 2017-12-12 Completion Testing No Violations On the date of my inspection I found a well maintained lease road with an unlocked gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with an unlocked gate. The pad has 3 wells, 4 sand separators, 3 gas production units, and two 500 barrel tanks within a metal dike. Since the time of my last inspection, Ascent Resources has drilled 2 more wells. Permit number 3408120654 is currently shut. Pressure gauges on the surface and intermediate casing on this well currently read 0 and 3100 pounds per square inch respectively. ESS Corporation is currently onsite with SEI working on the new production equipment. There is also some miscellaneous equipment on the pad related to the equipment installation activities. The Ascent company man indicated that he thought frac operations would begin on the 2 new wells within 3 weeks. The 2H will remain shut in until they flow back the 4H and 6H.

1509009448 2017-10-26 Casing String Witnessed No Violations I am on site today to witness the cementing of the 18.625 inch mine string casing. Patterson Drilling rig 292 drilled a 24 inch diameter hole to a depth of 296 feet and set 292 feet of 18.625 inch, 87.5 pound per foot casing. Schlumberger pumped 35 barrels of gel then 656 sacks of class A cement, 1.19 yield, 15.6 pound per gallon, surface set. A coal seam was identified from 239 feet to 249 feet, two baskets were set at approximately 225 feet. Schlumberger displaced with 77 barrels of fresh water to land the plug. Cement did not returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 620 psi for 5 minutes. Patterson ran one inch tremie pipe down the backside of the 18.625 inch casing. Schlumberger pumped 8 barrels (38 sacks) of class A, 1.18 yield, 15.6 pound per gallon cement to set the basket. After waiting approximately 3 hours Schlumberger pumped an additional 27.2 barrels (128 sacks). Then after waiting another 2.5 hours, they pumped 45.5 barrels (215 sacks), cement returned to surface and remained static.
1615866397 2017-09-28 Drill / Deepen / Reopen No Violations On the date if my inspection I found that the conductor has been set for this permit number. The cement has been brought to approximately 6.5 feet below ground surface. The cellar was drilled to 8 feet, which leaves approximately 18 inches of good cement in the floor.
