0106966234 2017-07-14 Completion Testing No Violations On the date of my inspection I found Keane Frac in the process of hydraulically fracturing all three wells on this pad. I spoke with the Ascent company man who indicated that the operations were going smoothly and that they have completed stage 13 of 46 frac stages for this well. He indicated that they should be on this location for approximately 2 more weeks. All hazardous materials are within double containment which is in good condition. The containment around the hydrochloric acid tanker has a few punctures that the company man said he would have fixed as soon as possible. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position. The company man indicated that any water that accumulates within the containment is removed using a vac truck then transferred into the water holding tank on site and used for the fracturing process.
0330846787 2017-04-03 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1570 feet and set 1552 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1520 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 100 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1200 psi for 5 minutes.

0469471728 2017-08-03 Completion Testing No Violations On the date of my inspection I found the Keane Frac has finished stimulating the well and has removed all of their equipment from the pad. Currently there are containment crews on site patching holes in the containment in preparation for Deep Well Services. Deep Well Services is moving a work over rig onto the pad to drill out the 3 wells on the pad in preparation for production. The Ascent company man estimates drill out to take approximately 3 days per well. Falcon EFS flow back services is also currently on site rigging up. Pressure gauges on the surface and intermediate casing show 0 and 850 pounds per square inch respectively.
0588418070 2018-01-10 Preliminary Restoration No Violations On the day of my inspection I found this pad equipped with 3 producing wells. Production equipment includes three gas production units, three vertical sand seperators, and two 400 barrel produced water tanks. Produced water tanks are in steel containment and there are also two frac tanks in temporary containment. There is also Mark West pipeline equipment onsite. Production numbers were submitted for 2017. Surface casing has 7 psi and intermediate casing has 1123 psi. Site meets requirements of ORC 1509.072A for preliminary restoration.
0845364864 2017-03-20 Casing String Witnessed No Violations I am on site today to witness the cementing of 24 inch conductor casing. Precision Drilling drilled a 30 inch diameter hole to a depth of 125 feet and ran 125 feet of 24 inch diameter 96 pound per foot conductor casing. A coal seam approximately 10 feet thick was identified from 70 to 80 feet, a basket was ran at approximately 55 feet. O-Tex pumped 210 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal. O-Tex then displaced with 37 barrels of water to set the shoe. O-Tex ran 40 feet of 1 inch tremie pipe down the annulus and pumped 150 sacks of cement. Approximately 5 barrels of good cement returned to surface and remained static.
1472865977 2017-08-24 Completion Testing No Violations On the date of my inspection I found SEI Oilfield Services onsite assembling the production equipment for this pad. They are currently assembling the flowlines and gas production units. This pad has 3 wells that are currently shut in, awaiting flowback operations. I could not find an Ascent representative onsite to determine when flowback operations would begin. The pad is surrounded by healthy vegetation and the pad drains are in the closed position and do not appear to be leaking.
1491078358 2017-04-01 Casing String Witnessed No Violations The purpose of my inspection was to witness the hydrostatic pressure casing test on the conductor/mine casing string on well #081-2-0593. Company Representative Eugene Lanham reported drilling rig Falcon #26 tagged the top of cement at 88 Kelly Bushing (KB). Falcon #26 drilled a 17.5 hole from the top of cement to a total measured depth of 148 KB. The 24 conductor/ mine casing string was set at 125 below ground level. After tripping the drilling assembly out of the bore hole, a vacuum water truck transferred approximately 50 bbls of fresh water through the gravity method into the well. Surfactant utilized in the drilling process prevented accurate observation of the water level within the casing. I requested that the casing be topped off with additional water for the test. Additional water was added to displace the foam into the cellar. After displacing the foam, I observed the water level for thirty (30) minutes and the static water level did not fall. I communicated to Mr. Lanham that the hydrostatic test passed and operations could proceed as planned.
1735630801 2017-05-30 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Pioneer drilling rig 77 is currently running their cement bond log before running their 9.625 inch intermediate casing to 8614 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. The containment areas appear to be in good condition free of tears or punctures. The area near the half round that tested high for chlorides on 5/9/17 and 5/15/17 remains contaminated. The dimensions of the area are approximately 25 feet wide by 70 feet long. Today the chlorides tested for greater than 6412 parts per million using Quantabs. The Chesapeake company man, Ralph, was with me during the field test and indicated that he would discuss the matter with the superintendent.
