0434765088 2017-05-09 Drill / Deepen / Reopen No Violations I am on site to perform a walk around inspection and status check. At the time of my inspection, Pioneer rig 77 was beginning to run 8618 feet of 9.625 inch casing. During the walk around inspection of the location I found containment around one drum of Thermguard to have a 1 inch hole. The Ascent company man was notified and he said he would have the drum moved to a different containment area. I also found an area on the west side of the pad near the containment berm to be contaminated with brine. 4 puddles within a 20 foot by 80 foot area tested for greater than 6412 parts per million chloride content. The Ascent company man was notified and indicated that they would bring in fresh water trucks and flush the brine through the pad drain system. The flush water would be collected at the catch basins and hauled off site for disposal. I asked Ascent to notify me when the cleanup process was complete.
0861479132 2017-09-15 Production Wells No Violations
1135196367 2017-08-03 Completion Testing No Violations On the date of my inspection I found the Keane Frac has finished stimulating the well and has removed all of their equipment from the pad. Currently there are containment crews on site patching holes in the containment in preparation for Deep Well Services. Deep Well Services is moving a work over rig onto the pad to drill out the 3 wells on the pad in preparation for production. The Ascent company man estimates drill out to take approximately 3 days per well. Falcon EFS flow back services is also currently on site rigging up. Pressure gauges on the surface and intermediate casing show 200 and 500 pounds per square inch respectively.
1228138963 2017-05-15 Drill / Deepen / Reopen No Violations I am onsite to perform a general site inspection. Pioneer drilling rig 77 is currently drilling the production string at approximately 12,380 feet. The pad is in good condition and the vegetation on the berms appears healthy. The pad drain valves are closed and do not appear to have any leakage issues. Containment appears to be in good condition with no apparent holes or tears. All hazardous materials are within containment areas. Standing water near the brine contaminated area from the previous inspection on 5/9/17 tested 2619 parts per million for chlorides. I notified the Ascent company man about the issue and he said they would flush the area with fresh water again.
1490017755 2017-03-17 Casing String Witnessed No Violations I am on site today to witness the cementing of 24 inch conductor casing. Precision Drilling drilled a 30 inch diameter hole to a depth of 125 feet and ran 125 feet of 24 inch diameter 96 pound per foot conductor casing. A 10 foot mine void was found from 70 to 80 feet, a basket was ran at approximately 55 feet. O-Tex pumped 230 sacks of class H cement, 1.2 yield, 15.6 pounds per gallon with 2 percent Calcium Chloride and 0.25 pound per sack flow seal to set the shoe. 1 bag of calcium chloride, 1 bag of cotton seed hulls, and 1 bag of gravel were poured down the backside of the 24 inch casing into the basket. O-Tex ran 10 feet of hose down the hole on one side of the casing to pump cement and ran another hose down the other side to pour approximately 10 gallons of Sodium Silicate to flash set the cement in the basket. O-Tex pumped approximately 130 sacks of cement until it returned to within 10 feet of surface and remained static. Ascent will come back and top out with ready-mix.
1490659107 2017-03-27 Drill / Deepen / Reopen No Violations I am onsite today to witness the hydrostatic test for the 24 inch diameter conductor mine string combination. Falcon Drilling rig 26 drilled to 140 feet, 15 feet beyond the last total depth of 125 feet. Fresh water was pumped into the 24 inch casing and brought to the top. When pumping stopped, the water level fell to approximately 80 feet. This is the same approximate depth of the bottom of the mine. The hydrostatic test failed. Ascent plans to blow the hole dry and pump a balanced plug of 250 sacks followed by 5 barrels of water.
1491203722 2017-03-31 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Falcon Rig 26 drilled a 17.5 inch diameter hole to a depth of 1565 feet and set 1551 feet of 13.375 inch 61 pound per foot casing. O-Tex established circulation with fresh water, pumped 1520 sacks of class H cement, 1.2 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 100 barrels of cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 1200 psi for 5 minutes.
1515592611 2018-01-10 Preliminary Restoration No Violations On the day of my inspection I found this pad equipped with 3 producing wells. Production equipment includes three gas production units, three vertical sand seperators, and two 400 barrel produced water tanks. Produced water tanks are in steel containment and there are also two frac tanks in temporary containment. There is also Mark West pipeline equipment onsite. Production numbers were submitted for 2017. Surface casing has 52 psi and intermediate casing has 502 psi. Site meets requirements of ORC 1509.072A for preliminary restoration.
1666247832 2017-08-24 Completion Testing No Violations On the date of my inspection I found SEI Oilfield Services onsite assembling the production equipment for this pad. They are currently assembling the flowlines and gas production units. This pad has 3 wells that are currently shut in, awaiting flowback operations. I could not find an Ascent representative onsite to determine when flowback operations would begin. The pad is surrounded by healthy vegetation and the pad drains are in the closed position and do not appear to be leaking.

Falcon Drilling rig 26 drilled to 150 feet, 10 feet beyond the last total depth of 140 feet. Fresh water was pumped into the 24 inch casing and brought to the top where it remained static for 30 minutes.

1918831655 2017-07-14 Completion Testing No Violations On the date of my inspection I found Keane Frac in the process of hydraulically fracturing all three wells on this pad. I spoke with the Ascent company man who indicated that the operations were going smoothly and that they have completed stage 13 of 46 frac stages for this well. He indicated that they should be on this location for approximately 2 more weeks. All hazardous materials are within double containment which is in good condition. The containment around the hydrochloric acid tanker has a few punctures that the company man said he would have fixed as soon as possible. The location is surrounded by a fenced berm with healthy vegetation. The pad drain valves show they are in the closed position. The company man indicated that any water that accumulates within the containment is removed using a vac truck then transferred into the water holding tank on site and used for the fracturing process.
