0311298590 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 0 psi surface, 650 psi intermediate, 2100 psi tubing, 1200 psi flowline, and 1500 production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
0398336202 2015-05-11 Drill / Deepen / Reopen No Violations On the date of inspection I am on site to check the status of construction. Anderson Excavating is in the process of cutting the pad down to grade and installing and compacting lifts along the fill slopes. Erosion and Sediment Controls are in place.
0416569849 2017-03-19 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Nomac Drilling rig 84 drilled a 24 inch diameter hole to a depth of 378 feet and set 378 feet of 18.625 inch, 87.5 pound per foot casing. Schlumberger pumped 189 sacks of class A cement, 1.18 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. A coal seam was identified from 321 feet to 328 feet and two baskets were set at 315 feet and 305 feet. No cement returned to surface. Schlumberger pumped 5 barrels of class A, 1.2 yield, 15.6 pound per gallon cement through approximately 240 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket. After setting for 2 hours, Schlumberger pumped another 5 barrels. Cement levels were rising enough to continue pumping. 6 top outs were required to bring cement to surface. A total of 357 sacks were used for the top out before cement was returned to surface.

The pad is surrounded by an earthen dike with healthy vegetation and a fence with an open gate. The pad has 3 cellars with set conductors in them. The conductors were set in 2015. The pad is currently being used to store haul trucks.
1687018097 2015-06-22 Casing String Witnessed No Violations On this day Precision drilling drilled the 36 open hole to a depth of 100. The 30 conductor casing was ran and cemented by Allied Cement Company. They ran 308 sks of class A cement and cement was circulated to surface. See cement ticket for details.
1754409627 2017-03-21 Casing String Witnessed No Violations I am on site to witness the running and cementing of 13.375 inch surface casing. Nomac Drilling Rig 84 drilled a 17.5 inch diameter hole to a depth of 1645 feet and set 1644 feet of 13.375 inch 61 pound per foot casing. Schlumberger established circulation with fresh water, pumped 1365 sacks of class A cement, 1.18 yield and 15.6 pound per gallon, 2 percent calcium chloride, 0.25 pound cello flake per sack. Approximately 53 barrels of good quality cement returned to surface. The wiper plug landed and pressure was increased to approximately 500 pounds per square inch (psi) over pump pressure, the casing held approximately 918 psi for 5 minutes.
