0156454139 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 0 psi surface, 900 psi intermediate, 2100 psi tubing, 1600 psi flowline, and 1600 production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.
0209814446 2017-02-23 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Nomac Drilling rig 84 drilled a 24 inch diameter hole to a depth of 378 feet and set 378 feet of 18.625 inch, 87.5 pound per foot casing. Schlumberger pumped 175 sacks of class A cement, 1.18 yield, 15.6 pound per gallon, 2 percent Calcium Chloride. A coal seam was identified from 324 feet to 331 feet and two baskets were set at 310 feet and 282 feet. No cement returned to surface. Schlumberger pumped 5 barrels of class A, 1.18 yield, 15.6 pound per gallon cement through approximately 240 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket. After setting for 2 hours, Schlumberger pumped 5 barrels of class A, 1.18 yield, 15.6 pound per gallon cement through approximately 240 feet of tremie pipe on the backside of the 18.625 inch casing. This was repeated for a total of 4 times. On the fifth top out, cement was returned to surface. The top out cement totaled 84 barrels. Cement was brought to surface where it remained static.
0443029000 2017-03-08 Drill / Deepen / Reopen No Violations Today I am on site to perform a walk around inspection of the drilling operations. I was accompanied by the company man for Ascent. Nomac 84 is currently tripping in the hole to begin building the curve for the lateral portion of the well. Upon inspection I found multiple breaches in primary containment surrounding their chemical storage area, as well as torn bags of calcium chloride that had partially dissolved from the recent rain. Heavy rain from the last few days has flooded this area of the pad. The water which is still contained within the pad berm cannot be drained via the pad drains as it failed a chloride test administered by the water management contractor Ascent uses. They are currently using vac trucks to haul the water away.

The pad is surrounded by an earthen dike with healthy vegetation and a fence with an open gate. The pad has 3 cellars with set conductors in them. The conductors were set in 2015. The pad is currently being used to store haul trucks.

Nomac Drilling rig 84 drilled to 389 feet, 11 feet beyond the last total depth of 378 feet. Fresh water was pumped into the 18.625 inch casing to surface and held static level for 30 minutes.

Quantabs tested the water at approximately 38 parts per million. The sample had a pH of approximately 7.
