0947665516 2018-03-14 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeled producing wells. Well is equipped with christmas tree style wellhead. Gauges on wellhead read 0 psi surface, 1300 psi intermediate, 2600 psi tubing, 1350 psi flowline, and 1800 production. Production equipment includes three line heaters, three gas production units, two 500 barrel steel tanks, and 6 sand seperators. Pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.

1181805532 2015-06-22 Casing String Witnessed No Violations On this day Precision drilling drilled the 36 open hole to a depth of 100. The 30 conductor casing was ran and cemented by Allied Cement Company. They ran 308 sks of class A cement and cement was circulated to surface. See cement ticket for details.

1485182144 2017-01-23 Casing String Witnessed No Violations I am on site today to witness the running and cementing of 18.625 inch mine casing. Nomac Drilling rig 84 drilled a 24 inch diameter hole to a depth of 377 feet and set 360 feet of 18.625 inch, 87.5 pound per foot casing. Schlumberger pumped 706 sacks of class A cement, 1.19 yield, 15.9 pound per gallon, 2 percent Calcium Chloride. A coal seam was identified from 320 feet to 328 feet and two baskets were set at 305 feet and 300 feet. No cement returned to surface. 3 bags of cello flake were poured into the baskets along with 150 pounds of calcium. Schlumberger pumped 6 barrels (31 sacks) of class H, 1.03 yield, 17 pound per gallon cement through approximately 240 feet of tremie pipe on the backside of the 18.625 inch casing to set the basket. After setting for 2.5 hours, Schlumberger pumped 43 barrels (230 sacks) of class H, 1.05 yield, 16.5 pound per gallon cement through approximately 240 feet of tremie pipe on the backside of the 18.625 inch casing. Ascent shut the job down when the cement stopped coming up. After setting for 3 hours, Schlumberger pumped 20 barrels (107 sacks) of class H, 1.05 yield, 16.5 pound per gallon cement through tremie pipe on the backside of the 18.625 inch casing. After setting for 4 hours, Schlumberger pumped 63 barrels (302 sacks) of class H, 1.17 yield, 15.8 pound per gallon cement through approximately 50 feet of tremie pipe on the backside of the 18.625 inch casing, approximately 18 barrels of good cement was returned to surface and remained static.
1485348043 2017-01-24 Drill / Deepen / Reopen No Violations Upon arrival on location I found the rig crew drilling out the cement and the float shoe. The well bore was drilled to a new depth of 384 feet. The hole was loaded with water and a hydrostatic test was completed with no loss of fluid. The test passed..
1485451553 2017-01-26 Drill / Deepen / Reopen No Violations Upon arrival on location I found that the well crew had drilled a 17.5 inch hole to a depth of 1659 feet. They then installed 13 3/8 inch casing. Schlumberger cemented the wellbore with class A cement, 1350 sacks. Circulation was established and followed with a 20 bbl. gel spacer and the followed with 283.5 bbl. of 15.5 lb. cement and displaced with 245.5 bbl. water. We had 65 bbl. of good cement returns to surface. Bumped the plug and held for 5 minutes. Floats held. Job complete.
1486058228 2017-02-02 Administrative Inspection No Violations I was contacted by Ascent Resources Superintendent Richard Thompson on 2/1/17 regarding lost tools down hole. Ascent had drilled a 12.25 inch hole to 5750 feet and were tripping out when they backed off the tools due to a tight spot. The bit depth was 5597 when the tools were lost. The tools were lost on 1/30/17, and after multiple attempts to retrieve them Ascent decided to leave them in the hole and set an 800 foot plug using Class H, 17.5 pound per gallon cement. The plug was set from the top of the fish at 5368 feet to 4568 feet.

2140735744 2016-12-06 Drill / Deepen / Reopen No Violations On the date of my inspection I found a well maintained access road with an unlocked gate. The pad is surrounded by an earthen dike with healthy vegetation and a fence with an open gate. The pad has 3 cellars with set conductors in them. The conductors were set in 2015. The pad is currently being used to store haul trucks.
