1428435496 2015-04-07 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the 9.625 inch second intermediate casing. I was informed by American Energy Utica consultant Mickey Brewer that the Nomac 83 rig had drilled a 12.25 inch diameter hole to a depth of 8544 feet. AEU set 9.625 inch, 40.00lbs/ft. casing at a depth of 8535 feet. Schlumberger pumped 750sk of Class A lead cement at 12.0ppg, 1.93 yield, 10.0% D154, 0.6% D046, 0.45% D238, 0.1% D065, and 0.67% D201. Schlumberger pumped 1380sk of Class A tail cement at 15.6ppg, 1.19 yield, 0.2% D046, 0.1% D065, 0.2% D153, 0.4% D207, and 0.49% D201. Schlumberger displaced cement with 466bbl of Oil Based Mud at 10.5ppg. 32bbl of cement returned to surface. The plug was not bumped but floats held. I informed Mr. Brewer that per ORC 1501:9-1-08(D)(3)(b), AEU would be required to pressure up on the cemented string to a value of at least 0.2 multiplied by the length of the casing string over 30 minutes with losses not exceeding 10 percent to verify casing and cement integrity.

-15 gallons of Oil Based Mud both onto and off of containment. The vacuum truck on site began sucking up the OBM that mixed with rain water around the pump truck. The rig crew set up absorbent booms and absorbent pads around the area to contain it in a low spot on the well pad. Mr. Brewer plans to continue the cleanup after the pump truck is able to move off of the contaminated area by using a vacuum truck and absorbent materials. If OBM is still present, Mr. Brewer plans to have an AEU construction manager scrape the rest of the material off the pad.

Precision set 100 feet of 30 inch diameter 108.00lbs/ft casing on bottom. Universal Well Services pumped 265sk of Type 1 cement, 15.6ppg, 1.20 yield, 2% Calcium, 0.25lbs/sk of flake. Universal dislaced cement with 62bbl of fresh water. 20bbl of cement returned to surface and stayed at surface while I was on site.
