0291780883 2014-11-23 Casing String Witnessed No Violations On the date of inspection I was informed by American Energy Utica consultant, Vernon Gallo, that Precision Rathole had drilled a 36 inch diameter hole to a depth of 100 feet. Precision set 100 feet of 30 inch diameter 108.00lbs/ft casing on bottom. Universal Well Services pumped 265sk of Type 1 cement, 15.6ppg, 1.20 yield, 2% Calcium, 0.25lbs/sk of flake. Universal dislaced cement with 62bbl of fresh water. 20bbl of cement returned to surface and stayed at surface while I was on site.

1419633491 2014-12-26 Casing String Witnessed No Violations On the date of inspection I am on site to witness the cementing of the mine string. I was informed by American Energy Utica (AEU) consultant, Eugene Lanham, that the Alpha Hunter 9 rig had drilled a 27 inch diameter hole to a total depth of 335 feet (all depths measured from Kelly Bushing with a height of 12 feet above ground). There was a mine void expected but solid coal was encountered from 192 feet to 199 feet. The rig crew set 18.625 inch diameter casing weighing 87.50lbs/ft to a depth of 322 feet. Baskets were placed at 177 and 182 feet. Allied Oil & Gas Services pumped 850sk of Class A cement, 15.6ppg, 1.18 yield, 2% CaCl2, and 0.25lbs/sk of Cello Flake. Allied displaced cement with 87bbl of fresh water. No returns were visible at surface, fluid was visible in the annular space approximately 60 feet deep. Plug was bumped, floats held. The rig crew ran 170 feet of 1 inch tubing just above baskets. After running a string and bolt into the annular space, AEU believed they had good cement on top of their baskets. The cement was allowed to set up for approximately one hour and then Allied was able to establish circulation with water, then switched to cement. Allied pumped 409sk of Class A cement, 15.6ppg, 1.18 yield, 1% CaCl2, and 0.25lbs/sk of Cello Flake. 5bbl of cement returned to surface and stayed at surface while I was on site. I requested that AEU drill 10 to 20 feet of new hole below the shoe and then establish circulation with fluid to verify that the cement below the coal seam had set up in place. According to an e-mail from the rig, they were able to establish circulation and circulate on the hole for 15 minutes with no losses.
158582525 2017-8-22 0:00: PW No Violations 0.50
1877145836 2015-05-26 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
