0800950168 2014-12-19 Casing String Witnessed No Violations On the date of inspection I was on site to witness the cementing of the First Intermediate string. I was informed by American Energy Utica consultant, Eugene Lanham, that the Alpha Hunter 9 rig had drilled a 17.5 inch diameter hole to a depth of 2063 feet. Wyoming Casing Crew set 13.375 inch diameter, 61.00lbs/ft casing to a depth of 2048 feet. Allied Oil & Gas Services pumped 1603sk of Class A cement, 15.6ppg, 1.19 yield, 2% CaCl2, and 0.25lbs/sk of Cello flake. Allied displaced cement with 306bbl of water. Plug was bumped, floats held, 56bbl of cement returned to surface.
0898222459 2015-05-26 Preliminary Restoration No Violations On this date I am on site to conduct an inspection on the well site for Preliminary Restoration. The well passes for Preliminary Restoration per ORC 1509.072(A).
1201740000 2015-05-08 Casing String Witnessed No Violations On the date of inspection I was informed by American Energy Utica Consultant Mickey Brewer that the Nomac 83 rig had drilled a 12.25 inch diameter hole to a depth of 8,570 feet. 9.625 inch diameter, 40.00lbs/ft casing was set to a depth of 8,555 feet. All depths are measured from the Kelly Bushing at a height of 25 feet above ground level. Schlumberger pumped a lead cement consisting of 775sk of Class A (TXI), 12.0ppg, 1.93 yield, 0.6% D046, 10% D154, 0.45% D238, 0.2% D065, and 0.55% D201. Schlumberger pumped a tail cement consisting of Class A cement, 15.6ppg, 1.19 yield, 0.2% D046, 0.2% D065, 0.2% D153, 0.4% D207, and 0.46% D201. Schlumberger displaced cement with 647bbl of 11.5ppg Oil Based Mud. Plug was bumped and floats held. 40bbl of spacer and 27bbl of cement returned to surface.
1202261488 2014-12-15 Casing String Witnessed No Violations On the date of inspection I was on site to witness the cementing of the mine string. I was informed by American Energy Utica (AEU) consultant, Larry Henry, that the Alpha Hunter 9 rig had drilled a 27 inch diameter hole to a depth of 330 feet (All depths are measured from the Kelly Bushing, 12 feet above ground). The rig crew ran 18.625 inch diameter, 87.50lbs/ft casing to a depth of 318 feet. The Alpha Hunter 9 rig encountered a coal seam from 189 feet to 199 feet. Baskets were placed at 179 feet and 174 feet. AEU elected to attempt to cement the string conventionally. Allied Oil & Gas Services loaded the capacity of the pipe plus 10bbl with fresh water and then gel and flake for the last 35bbl. Allied pumped 920sk of Class A cement, 15.6ppg, 1.2 yield, 2% CaCl2, and 0.25lbs/sk of cello flake. Allied displaced cement with 85bbl of fresh water. No cement or fluid made it to surface, plug was bumped, floats held. Well was shut in. Rig workers dropped five 5-gallon buckets of pea gravel and 3 bags of CaCl2 to open baskets. Allied pumped 28sk of Class A Cement and shut down for one hour to allow cement to cure. Allied pumped 372sk of Class A cement, cement briefly surfaced and then fell back down hole, out of sight. Allied had to make round trip to retrieve more cement. Allied pumped 94sk of Class H cement, shut down, waited for 2 hours for cement to cure. Cement was tagged at 123 feet with a bolt and string. Allied pumped 306sk of Class H cement, 5bbl of cement returned to surface and stayed at surface.
