-stage cement job. 62 centralizers were installed on the casing string. O-Tex was the cementing contractor. The stage 1 lead cement was mudscaled and O-Tex pumped 208.6 bbls of cement slurry at 12.0 pounds per gallon (ppg). The stage 1 tail cement was mudscaled at O-Tex pumped 80.8 bbls of cement slurry at 15.3 ppg. At 7:08 AM, the first stage plug was released and displaced with 637 bbls of oil based mud (OBM). The stage 1 plug bumped at 8:30 AM and pressure was increased 500 psi over for 5 minutes.The float held when pressure was released and 3.5 bbl of freshwater bled back to the pump tanks. The stage 2 DV tool opening cone was dropped at 8:44 AM followed by the installation of the stage 2 closing wiper plug. The opening cone was given 30 minutes to reach the DV tool and open the ports. O-Tex pumped 20 bbls of gel to clean out the ports and then the hole was circulated using the rig pumps for 4 hours allowing the first stage cement to set up. After rigging back up, the stage 2 cement was mudscaled and O-Tex pumped 320.9 bbls of cement slurry at 15.3 ppg. The second stage plug was dropped at 1:53 PM and was displaced with 349 bbls of OBM. The plug landed at 2:44 PM and closed the ports on the DV tool, sealing the concrete in the annulus. Approximately 128 bbls of cement returned to the surface. A bond log was run to verify cement.

1569132629 2014-11-12 Drill / Deepen / Reopen No Violations On the date of inspection I am on site to check the status of the conductor casing that was cemented the previous day (Veterans Day). The conductor was cemented and 10bbl of cement returns were reported by Express Energy Services. Cecil Lovel was the American Energy Utica consultant on site. W. Pol Contracting was on site performing modifications to the well pad. They are extending the southeast corner of the pad to allow more room for the Patterson 294 rig that will arrive in the next two weeks. I notified the ODNR Engineering team that the modifications to the well pad were being conducted.

-Tex was the cementing contractor. There was good fluid circulation throughout the cementing operation. A 30 bbl gel spacer was pumped ahead of the cement to condition the hole. The cement was mudscaled, and O-Tex pumped 297 bbls of cement slurry at 15.6 pounds per gallon (ppg). The plug was released at 11:14 PM and was displaced with 198.5 bbls of freshwater. Approximately 25 bbls of cement slurry returned to surface. The plug bumped the float collar at 11:47 PM and pressure was increased to 500 psi over and held for five minutes. The float held when pressure was released and 1 bbl of freshwater bled back to the pump tanks.

2061287542 2018-01-18 Preliminary Restoration No Violations On the day of my inspection I found the pad has 4 properly labeled producing wells equipped with christmas tree wellheads, 4 sand separators, 4 vertical separators, 4 line heaters and pipeline equipment. On a separate fenced area north of the well pad there are 2 shared 500 barrel tanks within a metal dike, a large vertical separator, and a gas production unit. The pad also has two steel plates covering cellars south of the producing wells. No pressure on surface or intermediate casing gauges. Production records are current throug 3rd quarter 2017. Site meet requirements of ORC 1509.072A for prelininary restoration.
