0055108775 2013-05-01 Surface Facility Construction No Violations I was onsite to check the progress of the construction of the Hess production facility onsite. There is a 400 barrel production barrel installed contained within a steel SPCC dike. There are separators and other production facility onsite as well. The gas from the location is being sold through a polyline, but the sales line is insufficient for the quantity of methane being produced. A workover rig is being brought on location within the week to work on the Hess NAC 3H-20 well. This well is in production but is currently choked back. There are no violations at this time.

They had a small show of gas around 7805 feet when entering the Queenstown. They expect to drill to the desired depth of 8415 feet tomorrow allowing them to run their 9 5/8 casing. There are currently no violations at this time.

They are permanent production lines from the wells to the facility so that the gas can be processed onsite before it could be sold to a pipeline. The facility has the capacity to be expanded if more wells are to be drilled on this location.
0998705034 2014-07-24 Production Wells No Violations Met with AEU representatives Kevin Eshbaugh and Corey McNutt to gain access to site. This well is currently shut-in. AEU reps stated that they are unable to flow the wells on this pad simultaniously due to pipeline restrictions and that they are alternating production on the wells. Large texas style wellhead with 1 400bbl tank, 3 phase seperator, line heater, and dehydrator are onsite.

The well is being flared while being flowed back in order to handle the excess gas that cannot be sold in the poly line that is currently on location. 1354043773 2012-11-27 Drill / Deepen / Reopen No Violations I stopped to check the status of H&P #287 in their drilling. They are currently at 426 feet of the 600 feet they are trying to drill. They are currently trying to pull out of the hole with the hammer bit do to the hole collapsing back in on itself. Once out of the hole they will go back in and try to continue drilling. There are currently no violations at this time.
1356971707 2012-12-27 Drill / Deepen / Reopen No Violations I stopped at this location to check the status of the squeeze cement job on the 9 5/8 casing. They are currently reaming the perforations and DV tool in order to set casing patches. A total of 3 patches 4 feet long each will be set in the casing by Saltel Industries to close off the DV tool and the perforations at the estimated depth of 4650. After the patches are set the casing will be tested to 6000 psi using the onsite Halliburton truck. There are no violations at this time.
1357575845 2012-12-27 Drill / Deepen / Reopen No Violations I stopped onsite to investigate a confidential complaint received in an email. The complaint stated that there has been oil based mud spilled on location several times and that it is entering draining ditches on the backside of the location. This is to have occurred around 12/11/2012. I investigated the drainage ditch and the area around the mud tanks on location and saw no signs of any spills or overflowing tanks from oil based mud. I asked to check the tickets from Mohawk Transport to check when oil based mud came on location. The tickets showed 11/28/2012 but the well was never drilling using oil based mud during the date the complaint was filed. I contacted Marcus Attaway, Hess completions supervisor, and there was a 1 barrel spill of oil based mud when drilling the 3H-20 well, which was reported. There were no documented spills on this site and I saw no signs of spills on location. During this inspection there were no spills on site and no violations.
1357928606 2013-01-08 Drill / Deepen / Reopen No Violations I was onsite to check the drilling sit while the rig was waiting on orders. They have a whipstock on location to sidetrack the well should that plan be approved and ordered. The rig was currently shut down and company men were about to make a hitch change. There are no violations onsite.
1360156257 2013-02-05 Administrative Inspection No Violations I was on location to pick up the Mohawk Transport tickets for the location. The rig was currently drilling ahead at 12000 feet.
1361891580 2013-02-25 Administrative Inspection No Violations I was onsite to check the status of the hydraulic fracturing of this well. Baker Hughes was onsite waiting for the 10th stage of perforations to be run. The job will include a total of 24 stages. Hess production was onsite to establish a selling point through a 4 flow line from a previous well located on the location. The process was going to resume in the mourning and they estimate that they will be on location until Saturday conducting the frac.
1362419911 2013-03-01 Administrative Inspection No Violations I was on location to check the status of the hydrallic fracturing of this well. They are currently stuck on the 9th of 24 stages in the frac schedule. They are having trouble drilling out the plug seperating the 9th and 10th stages. They expect to start the 10th stage early Monday mourning.
1363800380 2013-03-18 Completion Testing No Violations I was onsite to check the status of the hydraulic fracturing and completions of the NAC 4H-20 well. They recently completed their 24 stage frac job and are now preparing the well to be put into production. A flair stack is onsite and the flow back process has began. They plan on using a poly line from an adjacent well to sell gas in order to hold their lease with the North American Coal Co. The well will be substantially restricted due to the lack of sellable pipeline capacity. They will initially flair some of the excess gas until the established sale quantity can be established.
1363802773 2013-03-18 Drill / Deepen / Reopen No Violations I conducted a inspection in regards to complaint #4164 regarding spilling of oil based mud. The Mohawk mud tanks are still on location but have been thoroughly cleaned and set aside on site for removal. The complaint will be closed out when the tanks are removed and the site is seen to show no signs of oil based mud contamination, which it currently shows no signs at this time.
1366214137 2013-04-16 Administrative Inspection No Violations I was onsite to check and confirm that all Mohawk frac tanks have been removed from the Hess NAC well pad. The pad is free of any oil based drilling mud and pictures documenting the cleaning of the frac tanks have been received. Complaint #4164 can not be closed out.
1366219641 2013-04-16 Production Wells No Violations I was onsite to check the status of putting this well into full production. A Key workover rig was on location to fish out a packer and some tools left in the hole. They plan to complete the well in 2 days time and be offsite. A GIS crew is currently onsite spotting locations for the production facilities that will be placed onsite. There are no violations at this time.
1371739256 2013-06-18 Production Wells No Violations I was onsite to check the status of the production of this well. There have been some delays in getting the production facilities completed. There was an oversized valve in place that needed to be switched out in order to allow proper control of the facilities. They expect the production facilities to be completed tonight and the well to be put into a chocked back production tomorrow afternoon.
1373563902 2013-07-10 Production Wells No Violations I was onsite to check the status of the production wells on this location. The location has the production facilities completely finished and the wells are being sold into a poly line, causing the wells to be choked back from full production at this time. I also checked the annulus gauges of the wells and this well showed no signs of annular pressures. There are no violations at this time.
1435819218 2013-05-16 Preliminary Restoration No Violations I was onsite to pass preliminary restoration for this well. The well has been completed and is currently in a choked back production state. A production facility is being completed onsite for the well as well when the proper pipelines can be installed to sell the full capacity of the well. All of the drilling equipment has been removed offsite and the proper erosion controls have been installed and are established. At this time preliminary restoration can be passed.

1500936205 2013-08-07 Production Wells No Violations I was onsite to check annular pressures of the wells on this location. This well is currently producing. One of the annular gauges showed 50psi of pressure and the valve is leaking into the cellar full or water. The valve is slightly bubbling and I have contacted Hess to inform them of the small leak so that their well tender can take care of the issue.
1610381579 2012-11-30 Drill / Deepen / Reopen No Violations I was onsite to witness the cementing of the 20 inch surface casing. The well was attempted to be circulated with 1000 barrels of fresh water but no returns were seen. The Halliburton job expected 70 barrels of cement returns. There were no returns of any kind seen on the job. The well was bond logged and it showed that the cement was around 80 feet below ground surface, just below the base of the 30 inch conductor. It was decided that a cement basket be set at 70.5 feet and the job be topped off with cement by Halliburton.

1698205920 2014-11-05 Production Wells No Violations On the date of inspection I was on site to check the status of the temporary plugging operation for the well. I was informed by American Energy Utica consultant, Andy Ingram, that he planned to set two temporary plugs in the well for safety reasons so that additional wells can be drilled on the same well pad. Precision Rathole equipment is on site to begin drilling conductors after the plugs are set.
1780929206 2012-11-16 Drill / Deepen / Reopen No Violations I was on site to check the status of the conductor hole that is being drilled by Butchs. There have been several attempts to drill the hole to 120 feet but have failed because the hole keeps collapsing in on itself. Currently the hole was drilled and then filled with Halliburton cement to try to provide support for the drilling of the hole without cave in. They are planning on letting the cement cure for 12 hours before redrilling the conductor hole. There are no violations on the site.
1938688602 2018-06-20 UrbanProduction Wells No Violations On the date of my inspection I found this pad to contain three properly identified producing wells and one open cellar with an unidentified wellhead (permit 20499.) There are also three sand separators, two vertical separators, three line heaters, and a 400 barrel steel tank within a metal dike. Surface annular pressure is 400 psi, Intermediate annular pressure is 0 psi, production casing pressure is 0 and tubing pressure is 1250 psi. Production is up to date.
2000275105 2013-01-31 Drill / Deepen / Reopen No Violations I was on location to pick up the cement tickets from the cementing of the 7 casing. The rig is currently in the middle of their kickoff at 9300. They estimate the will be drilling another 6 days before they will TD the well. There are no violations onsite at this time.
2040154135 2013-03-04 Administrative Inspection No Violations I was on location to check the status of the fracturing of this well. They are still in their 9th of 24 stages. They are having trouble drilling out the plugs with the coiled tubing to move onto the 10th stage. They also had some cross contamination of their frac water. Baker Hughes is onsite testing the water that Hess has been remediating and chemically treating. They are also running the water through a 5 micron filter before any of it is used in the fracturing process. They plan to get back to fracing the well tomorrow. 4 Mowhawk/Hardrock Excavating mud tanks are still on location but are fenced off and not being used for any process onsite. They are awaiting orders on how to have the tanks taking off location. There are no violations at this time.
2102986100 2013-05-28 Production Wells No Violations I received an email from Hess stating that they were going to production testing/producing this well for several days before the production facilities onsite were completed. The onsite consultant informed me that they have a flare stack onsite to allow an extra gas or pressures that cannot be sent down the sales line to be safely flared off. The well needs to be produced in order hold the lease and the well will be sold in this condition until the production facility is completed and the gas can be proccessed and sold through the flowline.

It is currently stuck in the hole and they are putting 90k lbs of pressure trying to move it. The plan is to set the packer free or damage it enough and sheer the pins so that it will deflate on its own. There currently no violations at this time.
