There are no violations at this time and preliminary restoration can be passed at this time.

1516307208 2018-01-18 Production Wells No Violations On the day of my inspection I found the pad has 4 properly labeled producing wells equipped with christmas tree style wellheads, 4 sand separators, 4 vertical separators, 4 line heaters and pipeline equipment. On a separate fenced area north of the well pad there are 2 shared 500 barrel tanks within a metal dike, a large vertical separator, and a gas production unit. The pad also has two steel plates covering cellars south of the producing wells. No pressure on surface or intermediate casing gauges. Production records are current throug 3rd quarter 2017.

Had trouble getting the BOPs to pass tests because of leaking valves on the choke manifold, leaking flanges on BOPs, and rubber rings on the test plug. Finally got all tests on the BOP to pass. No violations at this time.
1692065309 2014-11-08 Administrative Inspection No Violations Received e-mail from American Energy Utica (AEU) consultant, Andy Ingram, regarding the temporary plugs set in the completed well. AEU elected to set temporary plugs in the existing well acquired from Hess in order to safely drill additional wells on the same well pad. Andy Ingram informed me that Blue Dot put a back pressure valve in the tubing hanger and nippled the tree down and installed a night cap on top. Blue Dot set tubing pack off plugs in 2 7/8 inch 6.5lbs/ft tubing at depths of 8,251 feet and 8,173 feet.
1793713319 2013-08-07 Production Wells No Violations I was onsite to perform and inspection of this well. The wellhead is surrounded by fencing and the annular gauges on the wells are functioning as they should. There is currently no annular pressures seen on the well. The production facilities for this well have also had a new fence put around the location for security. There are no violations at this time.
1823297899 2014-07-24 Production Wells No Violations Met with AEU reps Kevin Eshbaugh and Corey McNutt to access site. Large texas wellhead onsite is currently shut-in. 3 400bbl tanks, a line heater, and a 3 phase seperator are onsite. Operator is alternating production between this well and Cross Creek A wells because of pipeline restrictions.
