0452592116 2008-09-19 Production Wells No Violations A follow-up inspection revealed that the remediation efforts performed on tihis well site have been completed. New topsoil along with seed and straw have been applied to the impacted areas. Vegetative growth was observed around these areas. The earthen dike has also been repaired and the contamination and pollution removed. Run tickets for the contaminated soil have been received. Violations have been resolved.
1019168302 2008-08-28 Production Wells No Violations A follow-up inspection revealed seed, straw and new topsoil. Oily material has been removed from location inside earthen dike. The section of the dike that required attention, due to a trench cut along the side has also been repaired. An impacted area located at the bottom of the hill will need to be addressed.
1410808047 2014-09-15 Production Wells No Violations Associated with complaint #4798. The caller had complained of a well or tanks leaking into a pond and of a smell of gas. There was no evidence of contamination in any body of water at the site, there was some contamination in the dike. The caller resides in a rental on the property and his landlord is also the well owner. The two parties are currently involved in a dispute over the rental agreement. I informed the county inspector Kip Thompson of the issues at the dike, he will return at a later date to perform a production inspection. Close complaint.
1454913489 2008-08-08 Production Wells Violations Noted A complaint alleging that a company has been dumping brine onto the ground surface was forwarded by OEPA on 8/8/2008. The complainant was contacted by telephone on 8/8/2008. The complainant states that the company has been dumping brine onto the ground surface for some time. The complainant also states that they had been previously informed by the company that releasing brine onto the ground surface is allowed because of the minerals. The complainant confirmed that this activity was never witnessed. The complainant was unsure of the well ownership and details therefore, provided address location of their residential home. An inspection revealed a 110 (length) x 76 (width) kill area from the tank battery location. A trench had been cut, in the earthen dike, to allow fluid to flow out of the dike and over the hill side. No identification was observed at either the well or tank battery. An earthen dike will need to be placed around the well-head. The status of this well will need to be changed from ND to PR. The company was contacted by telephone and informed of the complaint received and inspection results. A telephone call was received from Mr. Danny Baker shortly after placing the initial phone call. He states that he owns the well, but operates under Nuttco Oil & Gas.
