0421457573 2014-07-21 Production Wells No Violations I received a call on 07/19/2014 from a Mrs. Cindy Donahey. She stated she lives in columbus and stated she belived the well is leaking. This came from the company that is overseeing the landfill . This company is Cummins Ritter. Mrs Donahey also stated she wanted the gates locked the close acceess to the tank battery. I contacted well owner John Seifert and asked who owned the gate. John Seifert stated he constructed the gate and he also owned the lock. The well is in compliane with no violations noted. I called Mrs Donahey as she requested me to inform her of my findings.
0914922775 2010-10-05 Production Wells No Violations The Ohio EPA forwarded a landowner inquiry, which included an oil and gas concern alleging well location contains unnecesary equipment. An inspection of the well revealed well owner has placed well back into production. The well was equipped with a pumping unit routed to tank battery location with earthen dike and liner containing (1) 100 bbl, (2) 50 bbl tanks and standing knockout tank. A fence surrounds the perimeter of well location with locked gate. Therefore, the well was observed from a distance of approx 50 and contained the necessary production equipment. Notified Ohio EPA of inspection results, as no violations were noted. Ohio EPA will relay this information to landowner.
0915509251 2009-08-06 Production Wells No Violations Attended a meeting at the well site location with Mr. Seifert, the well owner and Mr. Mike Sherron with the Ohio EPA to discuss proposed plans for the well, due to completion of landfill reclamation project. Mr. Seifert states that the well would be placed back into production. The tank battery location will be moved to location across the road from the well. Mr. Seifert was advised of setback distances relating to oil tank to traveled portion of the road, separator to tank, and 3 separation between storage tank. Mr. Seifert informed that earthen would be needed for tank battery and well, due to proximity of road and wetland. It was also recommended that locks/bull plugs be installed for valves to avoid potential vandalism.

1086091483 2004-05-27 Production Wells Violations Noted A complaint from the OEPA office in Logan was received on this well, the inpsection revealed oil in dike, and oily soil outside of dike, it appears tank has ran over. Called John Seifert owner of well he will get area cleaned up.
1247079744 2009-06-16 Production Wells No Violations A complaint was received alleging non-production. Ohio EPA representative requests plug or produce and states that the well status remains unchanged. An inspection revealed that the well is currently idle. The well is not equipped with a pumping unit, but still located on-site. Tubing remains inside the well bore with no rods. (2) 100 bbl tanks remain on location but not connected. Records show that the well produced 30 bbls of crude oil in 2006. The well is located on an abandoned landfill site. Therefore, it had been taken out of production, due to an Ohio EPA reclamation project being performed on property. Ohio EPA representative states that construction was done in 2006 and the reclamation ended 10/2007. After the inspection, an attempt to meet with Mr. Seifert at his office was not successful. A message was left with his wife to contact me, in regards to this well.

1992979223 2010-01-06 Production Wells No Violations An inspection revealed that the well owner, Mr. Seifert, has hired contractor to perform road bore. This work is being done to avoid installation of tank battery on property that had contained an old landfill property that has since been remediated and regulated by Ohio EPA. The attempt to perform road bore was not successful, due to Bedrock. The well owner states that they will now road cut to allow installation of production line. Time and date to be determined. Also noted is that the well is now equipped with a pumping unit.
2004645878 2019-03-06 Production Wells No Violations I conducted a status check on this well today. I could access the property as it was fenced off and locked. I observed the well from Clay Hill Rd. The well was equipped with a pumping unit and motor. The tank battery consists of (1) 210 barrel tank, (2) 100 barrel tanks, and a knockout tank within an earthen dike. My inspection revealed one of the 100 barrel tanks was leaking brine from the heater tube. I tested the water for chlorides with a Quantab. The Quantab test resulted in a reading of 10 Quantab units which is greater than 6215 ppm for chlorides. I did not observe identification at the tank battery. The last year for reported commercial production was 2012. Photos were taken of the well and tank battery. The well owner will need to complete the following tasks within the specified time frames in order to bring well into compliance: (1) Correct leak within 5 days and remove brine contamination. (2) Post identification within 10 days. (3) Plug well within 6 months. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. I left a voicemail message for Chris Schmelzer at 12:48 PM. Time of inspection: 12:25 PM.
