1259701810 2009-11-30 Drill / Deepen / Reopen No Violations Urbanized Area Pre-site inspection was conducted with Chieftain Energy, Mr. Ron McBride and Mr. Doug Kitchen. Reviewed well site and proposed locations for drilling and production. Discussed requirements and provided copies of rules pertaining to wells located in urbanized area.
2141060892 2019-01-07 Drill / Deepen / Reopen No Violations During my inspection I observed 7.63 inch casing approximately 16 inches above ground. The casing had a cap on top of it. I pulled the cap to see what the well had in it. I observed fluid at surface level with no other casings or tubing. A permit was issued to Chieftain Energy Corp on 12/4/2009 to reopen the well. The permit has since expired. I took two photos of the well. I contacted Doug Kitchen on 1/8/2019 regarding this inspection. Mr. Kitchen said he intends to reopen the well this year and that the well is currently plugged. Time of inspection: 11:35 AM. Weather: 45 degrees F, Overcast
