Mr. Benny Altier, Plugging/Restoration Contractor, at location performing work. The mixture included: Annual Ryegrass, Ryegrass, Kentucky 31 Fescue, Climax Timothy, Sweet Clover and Red Clover.
0596596541 2010-07-15 Administrative Inspection No Violations Mr. Jack Greene called today at 3:46 p.m. to say that a letter had been sent to the landowner regarding the restoration or waiver requirement for the lease road. He states there has been no response from landowner. If not willing to communicate they will restore section of road containing gravel. The gravel will be transferred to bottom portion of road.

More vegetative growth will be needed at well site to prevent further erosion. A 25 area over hillside will need to be addressed. This area is within close proximity to location where the pit had been excavated. The Final Restoration will be evaluated in the Spring. Sections of lease road show signs of vegetation. The section containing gravel will require a landowner waiver. The landowner would like to keep this surface.

1256317839 2009-09-30 Final Restoration No Violations An inspection of the Final Restoration was requested by the plugging company (Altier Bros.). The restoration is not due until 1/22/2009. During the inspection, it was noted that steel and plastic pipe remains on property. A 500 section of lease road has not been restored and found to contain gravel. Altier Brothers had been instructed to restore or provide landowner waiver for the road. To date, a waiver the DMRM has not received a waiver.

Altier Bros.states that they would dig down and install ID plate tomorrow.

1904325066 2009-04-28 Production Wells No Violations A complaint was received alleging contamination/pollution, flowline on the lease road, lease road eroding, and tank battery location. Met with the complainant at the well site. An inspection revealed that the alleged contamination/pollution was old paraffin that has broken down around the well head location. Railroad ties placed under pumping unit may have also contributed to this area too. Absorbent pads were found in a sealed trash bag near a shed. The second bag found was ripped and contained old pads that have broken down. The well owner will be plugging this well as, a permit for this activity has been issued. As a result, they have been making an attempt to remove flowline from this location. Explained to the complainant that final restoration requires removal of production equipment and that the company is showing initiative. Note: A second well (same lease different property owner) will remain in production therefore, the tank battery location will remain the same. The lease road contained a few ruts, but no significant erosion (area closer to twp road contains gravel), but will be graded after the well has been plugged. The earthen dike placed around the tank battery (front side) will also need to be built up, due to a nearby creek. No violations will be issued. Spoke with company representative by telephone and instructed to grade lease road after plugging and to build up earthen dike around tank battery.
1991110979 2010-06-16 Final Restoration No Violations An inspection revealed final restoration does not meet requirements. Mr. Jack Greene with Oxford Oil will be contacted regarding this inspection.
2014262888 2010-05-25 Final Restoration No Violations An inspection revealed that the final restoration does meet ORC 1509.072. Minimal vegetative cover was observed at the well site. Another application of seed will be recommended to the well owner. As previously noted, a section of the lease road above the tank battery location contained gravel with no vegetation (approx 500). A landowner waiver or lease road restored will be required. Two sections of black plastic approx 6-1 in length were observed near the well site.
