0094353457 2017-01-12 Production Wells No Violations I arrived at 0830 hours and found the protections that were in place prevented the oil from traveling further down the water course. Northwood contractor on site at 0945 hours. They began removing leaf debris that was contaminated with oil and replacing the absorbent diapers. They began the work at the termination of the spill and worked up stream. Oil had pooled at the point of release. This oil was removed prior to replacing the absorbent pads downstream of the point of release. Pictures were taken during my inspection.
0135303413 2017-01-11 Production Wells No Violations Northwood contractor Mike Corp had two employees on site today. The absorbent pads that were saturated with oil were bagged and removed. The leaf debris that was contaminated with oil was also removed. New pad were used to remove any remaining oil. New photographs were taken. They will be back on 1-12-2107 to continue with the clean-up.

Pads were replaced as needed. I have instructed the contractor to replace pads and booms as needed until such time the oil is removed.

0763582251 2017-01-09 Production Wells Violations Noted I received a call from Mike Corp who works for Northwood Energy. Mike said his pumper found a 2 inch steel flowline that had been damaged by a tree. The flowline allowed crude oil to leak from the crack in the line. The oil was found to have traveled down stream from the point of the leak for approx. 500 feet. The oil was trapped in ice, this made the clean up difficult. Booms and absorbent pads were deployed to catch any oil that would be released as the ice melts. Temperatures are predicted to be above freezing on 1-10-2017. Personnel will be on scene to monitor the oil clean up as the ice melts. Photographs were taken and shared with ODNR Emergency Response unit.

Rain today, I walked the creek and found no sheen of oil in the water. The absorbent pads and booms will be maintained until no oil is evident in the water course.

This well produces into a shared tank battery with API permit numbers 1557, 1618, 1645, 1772, 1748, 1663, 1677, and 1810. The tank battery consists of (1) 210 barrel tank and (1) 50 barrel brine tank. I did not observe identification at the well. The identification sticker at the tank battery was ripped and I could not observe the permit number. The well owner shall post identification at the well and tank battery within 10 days. I contacted Mike Corp on 3/7/2019 and advised him of my findings. Time of inspection: 2:50 PM. Weather: 22 degrees F, Sunny
1782758473 2019-04-09 Production Wells No Violations My inspection revealed that identification has not been posted at the well or tank battery. I will contact Northwood Energy regarding this inspection and advise that identification shall be posted. Time of inspection: 9:50 AM.
2019547661 2017-01-20 Production Wells No Violations Northwood contractor Mike Corp on site. Heavy rain overnight has moved the remaining oil downstream where absorbent material have caught the sheen of oil. The site will be checked on 1/23/2017 to determine if all the oil has been removed.
2051767315 2017-02-01 Production Wells No Violations I met with Wayne National Forrest personal at the well to go over the clean up plan due to the oil spill. Absorbent pads and booms are being replaced as needed. The source area of the spill continues to have pockets of crude oil that are being released during rain events. It was decided to maintain the course of action and monitor the site, replacing absorbent pads as needed.
