0052784295 2009-05-15 Production Wells No Violations A follow-up inspection revealed that contamined soil that had been located underneath the tanks has been pushed over to west side of the property. The approximate amount resembled a typical load of plugging clay and could be 12-14 tons. Altier Bros., the contractor, called today and stated that contaminated soil(s) were not hauled to the landfill yesterday, but taken to Mr. Paul Sinkovitzs house. Mr. Sinkovitz states that contaminated soils were not hauled to the landfill because he is broke. After consulting with Oil & Gas Supervisor, Mr. Robert Stonerock regarding this issue, Mr. Sinkovitz was instructed to perform Bioremediation to include horse manure, lime, seed, mulch and tilling. Recommended the application of gypsum to also assist with the chlorides. Mr. Sinkovitz agreed to perform the Bioremediation. Mentioned to Mr. Sinkovitz that the placement of the 100 bbl tank with knock-out next to powerline approx 3-5 away is very dangerous. He states that the line contains a breaker and would not pose any danger.

A sample was taken from the tank battery for analysis. I was accompanied by Southwest Region Supervisor Dan Goins and Inspector Kip Thompson.
0163181189 2009-04-16 Production Wells No Violations Inspect Report Cont From 4/16/2009: Oily soil was observed at the tank battery location and bare area around 2 fiberglass tanks placed behind the (3) 100 bbl tanks. The earthen dike was approx 72 (width) x 96 (length). The separation distance between oil production tank(s) does not meet the 3 requirement. Several open buckets (no lids) were found to contain old and fresh oil. The lease road revealed that it would need to be graded. The identification placed at the common tank battery was found to be incomplete. Mr. Paul Sinkovitz arrived at location. Mr. Sinkovitz confirmed that he knew about the contamination and pollution by stating I tried to get the pit pumped last Thursday, but my pumper, Benny, is not very responsible. Mr. Goins instructed Mr. Sinkovitz that he would have to remove all contaminated material and take to an Ohio EPA approved landfill. Mr. Goins further stated that run tickets would be required along with fresh soil, lime, seed and mulch. And the tanks would also have to be moved to allow removal of all contaminated material.

Inspector Andy Thomas issued a compliance notice on 1/25/2013 to Paul Sinkovitz as the well was in violation of ORC 1509.11, ORC 1509.22(A), and OAC 1501:9-9-05 (A)1. Production has not been reported on this well since 2010. The pollution and contamination at the well has not been cleaned up. I observed fresh oil contamination at the tank battery that may be due to a leak from one of the tanks. The well is not in compliance. The well owner is currently incarcerated. 0790413377 2009-04-23 Production Wells No Violations A follow-up inspection was conducted with Oil & Gas Supervisor, Mr. Robert Stonerock. During the inspection, it was noted that brine pits are not allowed, identification needed at the tanks, water needs to be kept out of the earthen dike placed around the well, lease road would need graded, and contamination inside buckets could be used for compost. The pit has been pumped however, brine still exists inside pit and contaminated material has not been removed from location. Mr. Stonerock utilized Quantabs and verified that chlorides remain in the pit. Mr. Stonerock informed Mr. Sinkovitz of the 250 mg/l Cl- maximum limit. Note: Production water is being placed back down the well through tubing. Mr. Sinkovitz states that he floats the oil out, which also alllows for him to continuously pump his well. The UIC section will be contacted regarding this inspection.
0920176869 2017-07-07 Production Wells No Violations The well consists of a pumping unit, 3-100 barrel tank, 2-50 barrel plastic tanks and 2-30 barrel plastic tanks. The tank battery is shared with 3407321388. There has been no change in the status of the well. The polish rod is dull and the pulleys are rusty. New photographs were taken of the pumping unit showing there has been no attempt to bring the well into compliance. The well is in violation of 1501 9-1-07 . The contamination has not been removed.

1036595228 2018-06-05 Production Wells No Violations My inspection revealed the well consists of a pumping unit and 2-100 barrel tanks. The purpose of my inspection was to determine if the well had been brought into compliance. The condition of the well has not changed since my last inspection 12.18.2017. I compared the pictures taken during that inspection and found no change in the status of the well. I check the containment dike next to the well using high range quantabs. The high range quantabs did not detect any chlorides. The well lacked a legible identification. There is no way to contact the owner as he is currently incarcerated.
1041096113 2008-09-29 Production Wells No Violations An inspection of the stream impacted from the oil spill was conducted, in cooperation with the Ohio EPA. During the inspection, it was noted that the stream contains approximately 400 feet of intermittent impact from the spill. The crude oil contained in this stream is weathered. An absorbent pad was utilized to verify the crude oil. The pad showed signs of oil. The Ohio EPA obtained samples from an impacted area for laboratory analysis.
1074597431 2008-07-28 Production Wells No Violations A follow-up inspection revealed non-compliance. During the inspection, it was noted that more work would need to be performed. The contractor performing informed the DMRM that he was instructed not to remove the soil around the well location. Mr. Paul Sinkovitz had been instructed to remove the contaminated soil from this location. It was found that the soil had only been mixed and covered. Mr. Sinkovitz was again instructed to remove soil and provide run ticket for the landfill. During the inspection, it was noted that an earthen dike had been built around the tank battery and trench. Mr. Sinkovitz was informed that he would need to meet the 50 setback distance. Mr. Sinkovitz states that he does not have the money to move the tanks. Mr. Joe Hoerst was contacted by telephone regarding this incident and the tank battery location. It was confirmed that Mr. Sinkovitz will need to bring the tanks into compliance.

1171025547 2007-03-13 Production Wells Violations Noted An inspection revealed that brine is being stored in a man-made trench located along the side and front of the tank battery. The trench located in front of the tank battery (closest to the road) is routed underground approximately 50 to a metal container to contain the ultimate disposal of brine. These trenches do not meet SPCC requirements and pose a threat to groundwater contamination. A bare area was also noted surrounding the well head. Quantabs showed 5ppm.

1222345769 2008-08-26 Production Wells No Violations A follow-up inspection was conducted with Mike Sherron with the Ohio EPA. During the inspection, it was noted that more oil would need to be removed from the ravine. Seed has been applied to the area around the well. The (3) 100 bbl tanks have not been moved to meet the 50 ft setback distance from the road. Once tanks have been moved an SPCC dike will also needed. Contamination and pollution still exists in the trench surrounding the tank battery and areas behind the tanks. Brine exists in this trench with oil sheen. Oil exists around the tanks. A 100 bbl tank has been installed at the tank battery location.
1240262413 2009-04-16 Annular Disposal Violations Noted The Division of Mineral Resources Management received a complaint alleging contamination and pollution, and safety concerns. An inspection was conducted with Mineral Resources Inspector II, Mr. Dan Goins. During the inspection, contamination and pollution was observed at the tank battery location. At this location, a pit was observed to contain fluid with dead insects on the surface. Also noted was that the smell from this fluid resembled brine. The pit measured approximately 20 (width) x 50 (length). Therefore, High Range Quantabs (Titrators for Chloride) were utilized at three locations. The first location was below a 100 bbl tank (tank closest to the pit) inside the trench. The Titrator showed a unit value of 7.6 which equals 4783 ppm (mg/l) Cl-. The last 2 locations were taken inside the pit. The Titrators at these locations reached their maximum unit value. Note: A unit value of 8.2 equals 6465 ppm (mg/l) Cl-. Report continued on next page.
1241699662 2009-04-23 Salt Water Injection Well Violations Noted A follow-up inspection was conducted with Oil & Gas Supervisor, Mr. Robert Stonerock on 4/23/2009. During the inspection, it was noted that production water was being placed back down the well through tubing. Mr. Paul Sinkovitz stated that he floats the oil out, which also alllows for him to continuously pump his well. Mr. Tom Tomastik, Geologist, Technical Support Services Section and Mr. Jeff Fry, UIC Field Supervisor were then contacted and stated that this operation is considered to be an illegal saltwater injection well. On 5/1/2009, an inspection conducted by Mr. Stonerock and Mr. Fry confirmed illegal saltwater injection well. Both Mr. Stonerock and Mr. Fry informed Mr. Sinkovitz, in the field, to disconnect lines to the well for this operation.

No effort had been made to bring the well into compliance since my last inspection on 12.17.18. The well consists of a pumping unit, 3-100 barrel tanks and 2 poly tanks. 1359641280 2013-01-28 Production Wells No Violations Accompanied on inspection by Brent Bear and Justin Smith. The tank battery consists of 3-100 barrel tanks, 2-1100 gallon poly tanks, and 2-2200 gallon poly tanks. Fluid inside the tank battery was tested with a high range Quantab and it showed a reading of 6.0 which is representative of 1664 ppm chloride. There is also a knockout tank on top of one of the 100 barrel tanks. There is a 3/4 inch line that comes from the brine tank and connects to the production tee on the tubing of the well. The valve for that connection is shut off at the well head and at the tank. There are 2 PVC lines that tie into one line at the well. There is an available connection at the well on the 4.5 casing to tubing annulus using 2 quick connects. More pictures were taken.
1362064769 2008-09-04 Production Wells No Violations A follow-up inspection was conducted. The tank battery distance was verified with a measuring tape. The (3) tanks measured 34 and 36 respectively. Mr. Sinkovitz had stated in an email that the tanks are 47 feet from the road. Also noted during the inspection was that the contamination and pollution inside the trench and around the tanks still remains.
1365040188 2008-07-25 Production Wells No Violations A follow-up inspection revealed non-compliance. Mr. Paul Sinkovitz states that a dresser coupling, on a steel flowline, had evidently been leaking. This is an additional source for the release of crude oil into the ravine and pond. Further stating that he would transfer through this line. This line appears to be routed from Permit #1573 to the tank battery location also utilized by Permit #1523. Mr. Sinkovitz states that he noticed a drop in pressure, but didnt think it was a problem. He states that this line will be abandoned. Assisted Mr. Paul Sinkovitz with the deployment of a skirted boom, in order to prevent further contamination from entering the pond.

Altier Bros., Contractor, states that 3 loads were taken to the landfill yesterday. The pit location has been filled in and two remaining 100 bbls have been moved to location next to 100 bbl installed last year. It was noted that a 100 bbl with knockout has been placed next to a powerline approximately 3-5 away. Currently, lines that allow placement of brine are not connected to the tanks. 1617531077 2008-09-08 Production Wells No Violations A follow-up inspection revealed non-compliance. The intermittent stream located in the ravine and the neighboring pond were inspected for contamination and pollution. Oil was observed, in the stream, at locations upstream below the well location and downstream in sections. The last area was observed downstream around a bend near an earthen dam (tributaries drain into the dam) which had been constructed, as a result of the spill to prevent future impact on neighboring pond. Note: The Division did not instruct Mr. Sinkovitz to build this dam. At the time of the inspection, no contamination was observed in the pond. Vegetative growth was observed around the well site. The brine located in the trench around the tanks has been covered with dirt. Oil still exists around the tank battery. Tank battery location remains the same.

1669988326 2009-04-16 Annular Disposal No Violations Met with Brent Bear to assist him with the inspection. The inspection revealed brine leaking from a tank or tanks in the tank battery, brine was collecting in a pit dug in the dike area. Mr. Sinkovitz( owner) arrived on site, explained to Mr. Sinkovitz of the violations that need to be corrected. Pit area and tanks will need to be pumped out and tanks removed from dike, then all contaminated materials hauled to EPA approved landfill and disposed of. Haul in fresh soil and rebuild dike area. Mr. Sinkovitz said brine has been leaking since 4-10-09, last Thursday and has not been able to get Benny Altier there to pump out tank. Brent Bear will submit Notice of Violation and handle follow-up.
1740038808 2009-05-12 Annular Disposal No Violations A follow-up inspection revealed that cleanup at the tank battery location has begun. Spoke with Altier Bros. Representative, Contractor, about contaminated soil removal and earthen dike specifications. According to Altier Bros., Mr. Sinkovitz had requested that the earthen pit be left and not filled in. He would like to move the two tanks (closest to the road) inside the pit. After consulting with Oil & Gas South Region Supervisor, Mr. Robert Stonerock, it was confirmed that the pit would need to be filled in with fresh topsoil, once the contaminated soil has been removed. The contaminated soil is being hauled to the landfill. Furthermore, a 100 bbl tank (located to the right side facing battery) containing a leak was removed from the location and taken to Mr. Paul Sinkovitz property. Note: The Hocking County Sheriffs Dept was notified by Mr. Brent Bear prior to the follow-up inspection.
1742059916 2013-01-23 Production Wells No Violations FOLLOW UP TO NOV # -2045896203 An inspection of the well and the tank battery reveals that they are in violation. The identification tag is not legible and there is pollution and contamination on the site. The pollution and contamination around the well head consisted primarily of brine. It was inside of a containment dike that surrounded the well and pumping unit. A high range quantab test showed a value of 9.4 which is representative of fluid containing more than 6797 parts per million (ppm) chloride. The EPA acceptable value is 250 ppm. There was also oily fluid there. There is a small pond downhill from the well that has oily scum on top of it. There is fluid outside of the containment dike for the well that I tested with a high range Quantab. It had a value of 5.6 which is representative of 1379 ppm chloride. There are numerous buckets full of oily fluid around the well site and tank battery. Production records have not been submitted for 2011. All contaminated material and fluids need removed from well site and tank battery, the identification tag needs to be legible, and production records need submitted. Weather conditions are very cold, and there is a lot of ice present near the well and tank battery.
1840851901 2008-07-22 Production Wells Violations Noted A landowner complaint was received alleging that an oil spill has occurred, in a pond, which has resulted in a fish kill. This complaint was forwarded to the DMRM by the OEPA. Arrived at the location and met with Mr. Mike Sherron, Ohio EPA. An inspection revealed crude oil in a neighboring pond located approximately 1500 feet downstream from the well site. At the time of the inspection, four bluegill, in the pond, were observed to be stressing. Oil was also observed in a culvert located to the east of the pond at a lower elevation. The culvert receives water from the pond and then drains into a nearby creek. Booms and absorbent pads were installed inside the culvert to contain the spill. The NW corner of the property revealed an intermittent stream routed through a ravine, which leads directly into the pond. Crude oil was observed in the stream. Further inspection upstream revealed that the stream branches off into 2 tributaries. Mr. Paul Sinkovitz states that he found the spill on 7/20/2008 and that a rainfall event had washed contamination and pollution off site. It is estimated that approximately 5 bbls of oil has been released. The first source location is the well. Mr. Sinkovitz states that the well pressured up due to parrafin which resulted in one of his clear plastic lines breaking. PVC and plastic is utilized around the wellhead. Oil was observed around the wellhead. A path of stressed vegetation and signs of oil was also found leading into the nearby ravine. The second location is the earthen trench placed around (3) 100 bbl tanks, which was found to contain brine and oil (old and new). Mr. Sinkovitz states that a clear plastic line broke at this location resulting in a release. Mr. Sinkovitz was instructed to begin cleanup immediately on the pond and ravine. 1935306551 2003-10-22 Annular Disposal Violations Noted An inspection of the well site and tank battery revealed dike around production tanks do not meet SPCC requirements, saw dust inside dike. Bare area between production tanks and well head, approx. 50ft x 100ft. Appears there has been an old brine storage pit or a leak from tank battery in the past. Only Permit# on well(pump jack) and not legible at tank battery. 1992606897 2018-07-16 Production Wells No Violations My inspection revealed that the well was equipped with a pumping unit and motor. The well did not show signs of recent production as the polish rod had a dull appearance. I observed a separator near the well that was not hooked up to the well. The area around the wellhead appeared to have a brine scald. I observed old and fresh oil contamination around the wellhead. I did not see any active leaks during the inspection. I did observe an identification sign on the ground at the well but I didnt observe complete identification at the tank battery. 2045896203 2013-01-23 Production Wells Violations Noted An inspection of the well and the tank battery reveals that they are in violation. The identification tag is not legible and there is pollution and contamination on the site. The pollution and contamination around the well head consisted primarily of brine. It was inside of a containment dike that surrounded the well and pumping unit. A high range quantab test showed a value of 9.4 which is representative of fluid containing more than 6797 parts per million (ppm) chloride. The EPA acceptable value is 250 ppm. There was also oily fluid there. There is a small pond downhill from the well that has oily scum on top of it. There is fluid outside of the containment dike for the well that I tested with a high range Quantab. It had a value of 5.6 which is representative of 1379 ppm chloride. There are numerous buckets full of oily fluid around the well site and tank battery. Production records have not been submitted for 2011. All contaminated material and fluids need removed from well site and tank battery, the identification tag needs to be legible, and production records need submitted. Weather conditions are very cold, and there is a lot of ice present near the well and tank battery.
2077651540 2008-09-05 Production Wells No Violations A follow-up inspection was conducted at the tank battery location. Mr. Paul Sinkovitz was at this location. Mr. Sinkovitz was asked about his tank battery measurement of 47 from the road per email, and when the contamination and pollution would be removed from this location. In regards to the measurement, Mr. Sinkovitz states that he just made this number up. Mr. Sinkovitz further states I will think about moving the tanks and will use the dike for containment how I so please. Contamination and pollution was observed around the tanks and also inside the trench. Photographs were obtained.
