Spoke with Mr. Doug Kitchen today, in the field, regarding the flowline burial for this well and the non-production from Permit No. 1206. Mr. Kitchen was asked how much more time he would need and states that line burial could be completed by 12/11/2009.
0330845545 2009-12-10 Administrative Inspection No Violations A follow-up administrative inspection, in the field, was conducted with Mr. Robert Stonerock, South Region Oil & Gas Supervisor. Reviewed and discussed location of line burial. Chieftain Energy has failed to meet deadline set by NOV, and Compliance Schedule. During a 11/30/2009 meeting, Mr. Kitchen agreed to an 12/11/2009 deadline.
0681121475 2011-05-02 Production Wells No Violations FOLLOW UP TO NOV # -979152060 Upon inspection it was observed that identification was not present on the well head or tank battery, all valves were closed at the well head and tank battery, there was no motor on the pump jack, the polish rod was dry and had oxidation on it, there was a cut in the flow line and the earthen dike was 2/3 full of water (there had been substantial rainfall in the previous days). Vegetation and amphibian life were also observed within the dike as well as an oil sheen on top of the water. The chloride content of the water in the dike, when tested with low range Quantab titrators revealed a reading of 3.6 which is 143 ppm(mg/L). When researching the well in RBDMS, there were no production records for 2010, and the well had not produced anything since 2007. Well needs to be identified, and placed back in production or placed in temporary inactive status. All fluids need to be removed from dike and hauled away. Production reports need submitted to Columbus for 2010. Failure to comply will result in forfeiture of bond. Photographs were obtained at time of inspection. Spoke with Doug Kitchen on 5/3/2011 at 5:03 PM and he stated that he has bills of sale for gas from this well.

1154608432 2006-07-19 Annular Disposal Violations Noted An inspection was conducted at the facility owned by Chieftain Energy Corp on 7/19/2006. During the inspection, it was noted that gas was leaking at the wellhead and no identification could be found. The area surrounding the tank battery showed signs of a spill, which may have affected a nearby creek. At the time of the inspection, oil was not observed upstream nor downstream of the spill. It was noted however, that a dike has been built. The ID located at the tank battery must be corrected and show the correct area code and permit number.
1239026239 2009-03-18 Annular Disposal Violations Noted A complaint was received alleging non-production and flowline burial. An inspection revealed that the well is idle. The well does contain production equipment to include a pumping unit, but a valve was observed closed. The well consists of just one flowline therefore, product from the tubing and casing would be routed through this line. Chieftain Energy obtained ownership of this well in 2001 from Mr. Paul Grim. From 2002 to 2005, RBDMS records indicate that there was no oil or gas production. Prior to 2002, the only years that production had been reported was 1985 and 1995. In both of these years, oil had been produced. In 2006, Chieftain Energy reported gas production. Currently, no statement of production of oil, gas and brine has been submitted for 2007 and 2008. Identification was not found at the well. A meter located near the tank battery contained a chart (no sign of production, which stated February 2009. A valve was observed closed on the gas line routed to a separator at this location. The 2 diameter plastic flowline from this well is routed through the pasture field to tank battery. The flowline will need to be buried. Oily soil and fluid were observed inside the dike.

1470185559 2009-10-13 Production Wells No Violations A follow-up inspection to Notice of Violation and Compliance Agreement Schedule composed on 7/14/2009 with a deadline of 10/1/2009 revealed non-compliance. Prior to agreement, the NOV deadline was 5/7/2009. Chieftain Energy has failed to bury the remaining flowline. Approximately 200 has been buried, but approx 500 still remains on top of ground located in pasture field. Observed meter with chart dated 9/30/09 showing gas production from this well.
1547044714 2019-01-09 Production Wells No Violations I conducted an inspection on this well today accompanied by landowner Barney Cook. My inspection revealed that the well was equipped with rods and tubing but no pumping unit. The well was not capable of production and did not show signs of recent production. This well produces into a shared tank battery with API permit number 1207. The tank battery consists of (2) 100 barrel tanks and a knockout tank inside an earthen dike. Identification was posted at the tank battery but not at the well. The last year for reported commercial production was 2016 in which Hocking Hills Energy and Well Service reported 54 barrels of oil. I took two photos of the well. The well owner/operator shall submit a written plugging schedule to Brandon Mitchell at 5880 Memory Road Zanesville, Ohio 43701-9553 within 30 days, per ORC 1509:13(B)(7), and plug the well within 180 days. The well owner/operator will be notified if the plugging schedule is approved or denied. Time of inspection: 8:55 AM. Weather: 28 degrees F, Snowing

1844187176 2018-02-27 Production Wells No Violations I received a request from Mr. Neil Hinton to determine the status of this well. The well consists of a pumping unit and 1-100 barrel tank. The well had a legible identification. Mr. Hinton is planning on building a structure on the property and wanted to know if the well would have any environmental issues associated with building on the property. The well is in compliance and I have contacted Mr. Hinton to relay this information.
