1156215764 2008-07-22 Production Wells No Violations An inspection revealed that the company is working on the dike. A 100bbl tank has been pushed back approximately 3 feet away from the lease road. New topsoil has been brought in to build up front section of the dike. Area behind the tanks dug deeper to allow for containment. It has been noted that the landowner does not want the tanks moved to a location closer to the well. He states that there are plans to build cabins on this particular piece of property. The terrain does not allow for an alternative location. Cleanup has been performed on the wetland, pond, and along roadside.
1216824777 2008-07-15 Production Wells No Violations A telephone call was received alleging that an oil spill had occurred on Roby Road. Arrived at the site location at 2215 hours. Mr. Mike Sherron with the Ohio EPA was on-site. During the inspection, it was noted that the load line on a 100bbl oil tank had been clipped. Tire marks were observed on the earthen dike next to this line. The tank battery is located alongside the lease road, which is now being utilized as a driveway by landowners. It was noted that the township had been grading Roby Road and appeared to have made attempt to turn around, on this gravel lease road. The grader was observed parked at the end of Roby Road. The oil from this tank traveled along roadside culvert into a wetland. From this location, oil drained underneath road via culvert pipe and traveled into a pond. The well owner has been contacted and cleanup will commence in the morning. At this time, it is unknown as to how much oil has been released.
1217255044 2008-07-16 Production Wells No Violations Met with company representative from Altier Brothers, Mr. Paul Altier, Township Trustee, Mr. Coy Lehman, and Mr. Mike Sherron, Ohio EPA to discuss cleanup. It was noted that 2ft of oil was lost from the 100bl tank. Therefore, it is believed that approximately 20 bbl of oil was lost. The tank battery is not 50ft from the road. The township trustee, Mr. Lehman requested that the tank battery be moved to a different location. Further stating that if tanks are not moved that the township would abandon the lines and tanks, due to a 16ft right-of-way. Mr. Lehman was informed that this particular issue would be a civil matter. He was informed that options would be discussed with the company including an alternative location. Furthermore, he was also informed that this lease road is now being utilized, as a driveway leading to residential dwellings. Witnessed company perform cleanup and containment.

Mr. Paul Altier states that the landowner does not want the tank battery placed on his property because they will be building cabins. Their were plans to move tank battery to a location closer to the well. The company was instructed to bring in more topsoil and build up the front section of the dike, and push tanks back further from the road. The company will also trench the back section of the dike deeper.
