0381806832 2018-09-05 Storage Well No Violations I met with the landowner Mr. Herb Beamer at the well. Mr. Beamer called on 9/4/2018 to inform me of a leak at the drip tank. During the inspection I did observe a small leak at the drip. I had to be within 2 feet of the leak in order to hear it. The identification was faded but legible. I contacted Jordan Hicks with Columbia Gas on 9/6/18 and advised him of the leak. Mr. Hicks said he would take care of it and correct the faded identification. Time of inspection: 2:40 PM. 0411571250 2016-03-07 Storage Well No Violations My inspection today is a status check on a natural gas storage well. During the time of my inspection there were not any violations and the proper identification was posted on the well head.
0649619217 2018-02-22 Storage Well No Violations My inspection revealed the storage well has been shut in. Trans Canada ran a packer down to approximately 800 feet. They also loaded the production casing with brine water to ensure the packer stays in place. I was not present during the running of the packer. I was told the information by Trans Canada supervisor Mr. Dan Talbert.

At 10:06 AM I called Trans Canada manager Mr. Richard Fuller and was not able to leave a message concerning when the well would be repaired.
1046338289 2018-09-28 Storage Well No Violations I conducted a compliance notice follow-up inspection regarding the compliance notice issued by Inspector Jimmy Wiles on 12/12/2017. Mr. Wiles stated that the company identified a casing collar leak on the production casing that allowed the annulus to equalize in pressure with the production string. The downhole issue was corrected when Columbia installed a packer in the well referenced in Inspector Kip Thompsons report on 2/22/2018. On 9/5/2018 I met with landowner Mr. Herb Beamer and conducted an inspection on the well. I advised Mr. Jordan Hicks of a small leak and faded identification on 9/6/2018. 1513025009 2017-12-08 Storage Well No Violations On the day of 11-28-2017, I received a phone call from the oil and gas emergency response team about a complaint from a landowner with a Columbia Gas storage well on his property with 714 psi annulus pressure on this gas storage well. I called and talked with the landowner Herbert Beamer and he stated that Columbia Gas personnel told him that the annulus pressure was 714 psi when it was lasted tested for pressure.

On the day of 11-30-2017, I talked with Dan Talbott of Columbia Gas Transmission and set a meeting with them on 12-8-2017 at their Sugar Grove office. On the day of 12-8-2017, I met with Dan Talbott and Brent Crisler of Columbia Gas Transmission and during the meeting it was acknowledged that the well had pressure on the annulus between the surface casing and production string of the well. Thoughts on what could be happening for the annular pressure to occur was a possible casing collar leaking in the production string. Some radiological tests were performed on the well prior to meeting and a temperature log did show a potential casing collar leak. Columbia Gas Transmission and I travelled to the well and reviewed the pressure on the well. The test of the production string pressure was 706 psi and the annulus pressure of the well was also 706 psi. The Columbia Gas Transmissions engineer released the pressure on the annulus and it was at .1 psi after 15-20 minutes. Columbia Gas Transmissions, Dan Talbott, was asked to monitor the annulus pressure over the next couple of day by the Columbia Gas Transmission engineer. The company stated that they may correct the problem with the pressure in the annulus by running a packer down the production string, isolating the casing collar leak.

On the day of 12-12-17, I contacted Columbia Gas Transmissions technician Dan Talbott to talk about the compliance notice I was issuing to them and about submitting a plan in next five days to correct the defective casing.
1516566371 2018-01-21 Storage Well No Violations I received a call from ODNR emergency response concerning a gas leak. I met with Mr. Herb Beemer who called in the complaint. Upon arriving at the wellhead, I used my Sperian gas meter to attempt to detect methane. The meter did not register any amount of methane. I did smell a faint odor of methane at the wellhead. I told Mr. Beemer I would contact Trans Canada concerning this problem.
1516628300 2018-01-22 Storage Well No Violations I received a call from ODNR emergency response on 1.21.18 concerning a gas leak. My report on 1.21.18 details what was found. I contacted Mr. Dan Talbert today who stated he would have a well tender check the wellhead for a leak.
