0248277412 2019-06-05 Completion Testing No Violations On the day of my inspection I was accompanied by Supervisor Eric Conrad. We witnessed a Profrac frac crew on location tied into the Deucker 10H, 210H, 8H, and 6H wells. Profrac is pumping stage 64/110. Stage 64 plug set at 15305 feet, with bottom perforation at 15289 feet and top perforation at 15169 feet. Profrac pumped 70.1 barrels of 7.5% HCl, 8423.9 barrels of clean fluid, 1576 barrels produced water, 198200 lbs of 100 mesh sand, and 132220 lbs of 40/70 mesh sand. Total sand pumped was 330420 lbs. Maximum treating pressure was 10666 psi and average treating pressure was 10224 psi. Maximum rate was 95.4 barrels per minute (BPM) and average rate was 95 bpm. Break down pressure was 7053 psi. Initial shut in pressure (ISIP) was 5231. Frac gradient of 1.07 psi/ft. The 13.375x9.625 annular pressure was zero and the 9.625x5.5 annular pressure is zero.

0448606978 2018-12-04 Drill / Deepen / Reopen No Violations On the day of my inspection I was accompanied by inspector Jeff Adams. We witnessed that Express Energy was onsite with two auger rigs. They are currently drilling this 26 inch diameter hole at a depth of 53.5 feet. They are planning on drilling to 120 feet and cementing casing on Thursday.

0980858376 2019-05-06 Completion Testing No Violations On the day of my inspection I witnessed a Profrac frac crew onsite. Profrac is currently tied into the 8H and the 210H wells. This well has not been opened up for frac. There are fourteen 350 barrel produced water tanks on site, within their own containment. The 13.375x9.625 annular pressure was 0 psi and the 9.625x5.5 annular pressure was 0 psi.

1482417918 2018-12-06 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augered a 26 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 216 sacks of class A cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. 30 percent excess ran. Cement was displaced with 39 barrels of fresh water. There no vac trucks on location so I was unable to confirm cement was returned to surface. There were good returns and expect that cement was returned to surface but cellar had 4 feet of water in it at the end of the job.

1549370906 2019-02-04 Casing String Witnessed No Violations On the day of my inspection, I witnessed the surface casing cement job. Patterson 580 rig drilled a 17.5 inch diameter open hole to a depth of 692 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. The rig ran 672 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot. Shoe track is 43.5 feet long. Haliburton was onsite with 675 sacks of class H cement. Cement weighed with mudscale at 17.0 ppg and yield of 1.071. 50 % excess of cement was ran in the open hole area. The cement was displaced with 98 barrels of fresh water. 30 barrels of gel was pumped ahead of the cement. 30 barrels of cement returned to surface and floats held with 1 barrel of fresh water returned into truck. Mix water has a pH of 7 and a temperature of 58 degrees Fahrenheit.
1551352354 2019-02-27 Drill / Deepen / Reopen No Violations Upon my arrival on location, I witnessed Patterson rig 580 drilling an 8.5 inch diameter hole at the depth of 16,100 feet. The projected total depth of this well is 25,080 feet. They should reach this depth in a couple of days. They will notify me when they are ready to cement the production string.
1557861064 2019-05-14 Completion Testing No Violations Upon my arrival on location, accompanied by ODNR Inspector Dylan Pendleton, I witnessed Pro Frac tied into the Deucher 10H. Stage 3 of 110 stages has a plug set at 24,511 feet with bottom perforations at 24,500 and the top perforation at 24,375 feet. They pumped 3,000 gallons of 7.5 % HCL, 10,138.2 barrels of clean fluid, 198,880 pounds of 100 mesh sand, and 132,540 pounds of 40/70. Total sand pumped was 331,420 pounds. Maximum treating pressure was 11,030 PSI, with average treating pressure of 10,793 PSI. Break down pressure was 8,035 PSI. Maximum pumping rate was 92.3 barrels per minute, an average rate of 87.3 BPM. Initial shut in pressure was 8,905 PSI. 14-350 barrels produced water tanks in their own containment. No produced water was used on this stage. Annular pressure on the 13.375 by 9.625 was 0 PSI and the 9.625 by 5.5 was 12 PSI.

Stage 34 plug set at 19830 feet, with bottom perforation at 19820 feet and top perforation at 19699 feet. Profrac pumped 68.6 barrels of 7.5% HCl, 9187 barrels of clean fluid, 839 barrels of produced water, 198280 lbs of 100 mesh sand, and 131900 lbs of 40/70 mesh sand. Total sand pumped was 330180 lbs. Maximum treating pressure was 11246 psi and average treating pressure was 10922 psi. Maximum rate was 95.3 barrels per minute (BPM) and average rate was 93.4 bpm. Break down pressure was 7706 psi. Initial shut in pressure (ISIP) was 5797. Frac gradient of 1.14 psi/ft. The 13.375x9.625 annular pressure is zero and the 9.625x5.5 annular pressure is 20 psi.
