0144035491 2019-04-08 Completion Testing No Violations On the day of my inspection I met with company men Willard Scott, Wallace Bossert, and Bub Ruckert. Stone rig #15 is onsite. They have finished drilling out the frac plugs in this well and have already ran 7907 feet of 2.375 inch diameter production tubing. Annular pressures on the 13.375 x 9.625 and the 9.625 x 5.5 are both zero psi.

1538402271 2018-09-28 Casing String Witnessed No Violations PAD ADDRESS: 34355 BOWERS RD., BOWERSTON 44695. On the day of my inspection I witnessed Express had augured a 26 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. CJ/Otex was onsite with 180 sacks of class A cement. They brought 30% excess for a 100 foot conductor. Company Man Neil Brown decided to displace to 115 and leave a 5 foot shoe. Cement weighed with mudscale at 15.6 ppg and yield of 1.19. Mix water tested 67 degrees Fahrenheit, pH of 7 and 140 ppm chlorides. Cement was displaced with 40.8 barrels of fresh water. No cement returned to surface. David Newman said they tagged cement at 26 feet below cellar floor on 9/28. It was then grouted to surface.
2046384803 2018-11-21 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 tripping out of hole to pick up 8.5 inch bit for curve and lateral. They drilled an 8.75 inch diameter hole to 6991 feet. 13.375 inch diameter surface casing was set at 637 feet. 9.625 inch diameter intermediate casing was set at 2025. Intermediate section was drilled with water and bentonite gel. Other wells on pad made significant water in this section while drilling on air. Estimated total depth is 15442
