0030921447 2018-03-16 Drill / Deepen / Reopen No Violations Pad address is 46800 Annapolis Rd., Hopedale, Ohio. On the day of my inspection I witnessed Express had augered a 26 inch diameter hole to a depth of 125 feet. They then ran 120 feet of 20 inch diameter casing weighing 89 pounds per foot. Express was also onsite to cement the casing with 230 sacks of class A cement with a yield of 1.18. Additives include 2% cacl and .5 pound/sack celloflake. Cement weighed 15.6 pounds per gallon with pressurized mudscale. Cement displaced with 39 barrels of fresh water. 10 barrels of cement circulated to surface.
0089728962 2018-04-17 Drill / Deepen / Reopen No Violations On the day of my inspection I found that Patterson 795 had reached total depth with an 8.5 inch diameter hole at 22919 feet. Wyoming casing is on location and just started running the 5.5 inch diameter production casing. Cement job anticipated early morning Wednesday.
0304805340 2018-07-13 Completion Testing No Violations On the day of my inspection I witnessed Halliburton frac crew on location. Surface annular pressure is 0 and intermediate annular pressure is 0. Stage 6 has been stimulated on this well.
0641353872 2018-06-20 Completion Testing No Violations On the day of my inspection I found this pad contained 3 unlabeled wells that have been drilled and not stimulated. Wells do not have well heads or frac stacks installed yet, only night caps. Production equipment includes three 400 barrel steel tanks inside steel containment, three sand seperators, and four GPUs (two are mislabeled as schiappa trust). Pad is vegetated and stable.
1065152854 2018-12-31 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 3 properly labeling producing wellheads. This well is producing out of the 5.5 inch diameter production casing using natural reservoir pressure, from both sides of the wellhead. Production equipment includes 6 GPUs, 6 sand seperators, 6 line heaters (not plumbed), and 3 400 barrel steel tanks in their own containment. Surface annular pressure is 0 intermediate annular pressure is 500. Site meets requirements of 1509.072A for preliminary restoration.

1491385445 2018-08-01 Completion Testing No Violations On the day of my inspection I was accompanied by DOGRM inspector Bob Roush. ID to company men Jacob Peiffley and Matt Wood. Halliburton frac crew is onsite. Surface and intermediate annular pressures are 0 psi. 10H is currently being stimulated and this well is next. Wireline is in the 8H. Stage 53/69 plug set at 12472, bottom perforation at 12452, and top perforation at 12291. Perforated with 5 guns, 6 shots, on 40 foot spacing.
1532609656 2018-07-26 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew was on location. No stimulation was occurring during this inspection as there was not sufficient water onsite for the job. Surface and intermediate annular pressures are both zero at this time. Stage 35/69 is next stage for stimulation on this well was perforated from 16052-15891. Plug set at 16072. 5 guns with 6 shots on 40 spacing used to perforate.
1823734023 2018-07-18 Completion Testing No Violations On the day of my inspection I witnessed Halliburton frac crew on location. Stage 17: Plug is at 19672, bottom perforation is at 19652, and top perforation is at 19491. 5 guns with 6 shots each are used to perforate on 40 foot spacing. Surface annular pressure is 0 and the intermediate annular pressure is 0. this is the enxt well to be fracked.
1985634982 2018-08-20 Completion Testing No Violations On the day of my inspection I witnessed Stone Rig 15 on location. ID to company man Wallace Bossert. This well has been drilled out and is awaiting flow back operations. Surface and annular pressures are zero and 600 respectively. Asked company man to clean up along the eastern berm on the inside of pad. There is orange stained limestone in places along the berm.
2007805698 2018-08-09 Completion Testing No Violations On the day of my inspection I witnessed that Halliburton has moved off of location. An aboveground storage tank and other miscellaneous tank remain. Badger Daylighting is on location cleaning tanks. Surface annular pressure is zero and intermediate annular pressure is 400. Wells have been stimulated but not had frac plugs drilled out.
