0140605272 2019-05-14 Work Over Reconditioning No Violations On the day of my inspection I was accompanied by Inspector Jeff Adams. During our inspection Conserv Inc. was swabbing the well. Company representative Bob Vandermulen said that Stone Rig #15 ran 8999 feet of 2.875 inch diameter tubing in the well over the weeekend. The 13.375x9.625 annular pressure is 80 psi and the 9.625x5.5 annular pressure is 140 psi.
0220866799 2018-07-26 Completion Testing No Violations On the day of my inspection I witnessed that the frac operator has moved off of the pad. Aboveground storage tank and 12 frac tanks remain onsite. Well connect is entering through the southern edge of the pad. No personnel onsite but fence is down and ground is excavated. Production equipment includes two 400 barrel steel tank inside containment, four sand seperators, and four GPUs. Wellhead gauges read 0 psi on surface, intermediate, and production casing. This well has not been drilled out or flowed back. Wellpad outslopes are stable and vegetated execpt for well connect disturbance.
0375087413 2018-05-04 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 drilling their lateral section at a depth of 18326 feet. Rig is running 12 ppg mud with no flare. Pad drains are shut. Rain is area last night and everything appears stable.

0757118125 2018-03-30 Casing String Witnessed No Violations On the day of my inspection I witnessed Express had augured a 32 inch diameter hole to 120 feet. 120 feet of 20 inch diameter conductor casing was run. Express was onsite with 471 sacks of class A cement. Additives include 2% CaCl and .5 lb./sack cello-flake. Cement weighed with mudscale at 15.6 ppg and yield of 1.18. Cement was displaced with 39 barrels of fresh water. 24 barrels of cement returned to surface. Express cement supervisor Brian Sparks and David Newman CHK superintenent both onsite.

1487939334 2018-08-20 Completion Testing No Violations On the day of my inspection I witnessed Falcon EFS flowback services onsite rigging up desanding units. ID to company man Brian Waid. Two wells are on location. These wells are set up to produce directly out of the 5.5 inch diameter production casing, on each side of the wellhead. There are six desanders onsite. Production equipment includes four line heaters, four sand seperators, four GPUs, and two 400 barrel steel tanks in their own containment.
1524642203 2018-04-25 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 580 drilled a 17.5 inch diameter open hole to a depth of 716 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. Wyoming casing ran 701 feet of 13.375 inch diameter casing, j-55, 54.5 lbs/foot. Casing was centralized with 17 centralizers. Shoe track is 41 feet long. CJ/Otex was onsite with 745 sacks of class H cement. Additives include 1% CaCl and 2% gypsum. Cement weighed with mudscale at 16.6 ppg and yield of 1.07. 50% excess ran in open hole. Cement displaced with 102 barrels of fresh water. 30 barrels of gel pumped ahead of cement. 30 barrels of 16.2 ppg cement returned to surface, and floats held with .75 barrels back.
1530895404 2018-07-06 Completion Testing No Violations On the day of my inspection I witnessed BJ services was rigged up on this well and permit 21544. Currently this well has a wireline in the wellbore to perforate stage 18. Top of perforation will be 18720 and bottom perforation will be 18881. There are five guns with 6 shots for a total of 30 perforations. Spacing is 40 feet apart. This stage will have 330000 pounds of sand pumped. Surface and intermediate annualr pressures are zero. Fresh water aboveground storage tank (AST) on location.
1531752427 2018-07-16 Completion Testing No Violations On the day of my inspection I witnessed BJs frac crew on location. Surface annular pressure is 0 and intermediate annular pressure is 0. Stage 50 is next. Wireline is going into the wellbore to shoot perforations from 12480 to 12320. Plug is at 12501. Well will be perforated with 5 guns with 6 shots each on 40 foot spacing.
1546281955 2018-12-31 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain two properly labeled wellheads. These wells are set up to produce directly out of the 5.5 inch diameter production casing, on each side of the wellhead, and are doing so on natural reservior pressure. There are five desanders onsite left over from flow back. They are dissconected from wells. Production equipment includes three line heaters (not plumbed in), four sand seperators, four GPUs, and two 400 barrel steel tanks in their own containment. Surface annular pressure is 50 and intermediate annular is 150. Site meets requirements of 1509.072A for preliminary restoration.
1553309707 2018-06-20 Completion Testing No Violations On the day of my inspection I found this pad to contain two wellheads equipped with frac stacks. Two other frac stacks are onsite not on wells. No frac equipment onsite other than twelve frac tanks. Production equipment includes four sand seperators, four GPUs, and its gauges show no pressure. Well has not been stimulated.
2012823283 2018-08-10 Completion Testing No Violations On the day of my inspection I found this pad has 2 wells that have been fracked. Wells have not had frac plugs drilled out yet. Pipeline contractor appears to be restoring pad berm from pipeline connection. Several guys on location building flowline. Surface and intermediate annular pressures are zero.
