0437107752 2017-12-27 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed that a Mid East Trucking crane had lost control of the topdrive section of derrick while trying to rig up. The section of the derrick landed over top of the 1h, 3h, and 5h wells that were previously producing. Chesapeake had set bridge plugs in these wells around 7000 feet and loaded above the plug. A needle valve was knocked off of the 3h well and the 1h and 5h were not touched. Pressure was released from the 3h well however according to Company man Danny Lambert no LEL readings registered on their monitoring equipment. This all happened around 22:30 on 12/26. On 12/27 at around 16:00, the crane which toppled over was released from the top drive section of the derrick. The Topdrive section was stabilized with another crane, cribbing, a tandem winch truck, and a pole truck. At around 17:30 the derrick was lifted off of the wells and laid safely off to the side. Replacement wellhead cages are already onsite and Chesapekae production personal were onsite to remove the cap with broken needle valve and replace it. 2 men were in a man lift that was struck by the falling derrick but walked away from the incident.
0445669266 2018-01-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 571 tripping out to the Clinton at 7000 feet to load hole with oil base mud and start their curve. Hole is currently drilled to a depth of 8513 with 8.5 inch diameter bit.
0631712618 2018-08-23 Production Wells No Violations Well has been drilled, stimulated, and drilled out. Well has not been flowed back yet. ID to facilities foreman Trevor Joilet. Current activity on pad is production equipment construction and installation. Drilling of 4 new wells on this pad started in december of 2017. They have not been flowed back yet. Call to Chris Huey production foreman stated that flowback should start on 8-30 for the 4 new wells and he hopes to bring the 5 existing wells back online around mid september. Production equipment includes four line heaters, 4 sand seperators, and 4 GPUs for the new wells. Existing wells have 5 sand seperators, 5 GPUs, 2 combustors, 3 compressors, and 7 400 barrel steel tanks in their own containment. All nine wells on the pad are shut in. Surface annular pressure is 0 and intermediate annular pressure is 620.
0687610064 2018-07-12 Completion Testing No Violations On the day of my inspection I witnessed Stone rig 15 drilling out plug number 56 of 94. Surface annular pressure is 0 and intermediate annular pressure is 50 psi. South and east of the wellheads some crystalization was noted on the pad. David Reese and Blake Riley are company men for Chesapeake. They were informed of the potential containment issue and said that would get it fixed and cleaned up right away. The area is dry and does not appear to have been a large volume of fluid that escaped primary containment.
1331643449 2018-01-09 Casing String Witnessed No Violations On the day of my inspection I witnessed the surface casing cement job. Patterson 571 drilled a 17.5 inch diameter open hole to a depth of 679 feet. Previous casing set and cemented was 20 inch diameter conductor casing to a depth of 120 feet. Wyoming Casing ran 654 feet of 13.375 inch diameter casing, j-55, 54.5lbs/foot and a 4 foot stickup. Casing was centralized with 15 centralizers. Shoe track is 39 feet long. Sclumberger was onsite with 96 sacks of class A scavenger cement. Cement weighed with mudscale at 15.6 ppg and yield of 1.8. They also had 662 sacks of cemcrete tail cement weighing 17 ppg and a yield of 1.08. 50% excess ran in open hole. Cement displaced with 95 barrels of fresh water. 25 barrels of gel pumped ahead of cement. 55 barrels of cement returned to surface, and floats held with 0.5 barrels back.
1331761973 2018-07-18 Completion Testing No Violations On the day of my inspection I witnessed Stone rig 15 onsite. They have drilled all plugs in this wellbore. Surface annular pressure is 0 and intermediate annular pressure is 150. Containment has been repaired from last weeks inspection and contaminated gravel was removed.
1369618537 2018-07-24 Completion Testing No Violations On the day of my inspection I witnessed Stone rig #15 in onsite. This well has had all plugs drilled out. Surface annular pressure is 30, intermediate annular pressure is 210, and production casing pressure is 2100.
2063443976 2018-01-02 Drill / Deepen / Reopen No Violations On the day of my inspection I am accompanied by ODNR emergency responder Jason Blocker. This is a follow-up inspection to the crane incident from last week. Patterson 571 has not gotten the damaged derrick back. They are currently setting the air package on location. Wellhead cages have been replaced with new, undamaged ones.
2143553583 2018-05-14 Completion Testing No Violations On the day of my inspection I found halliburton rigged up to the 6H, 206H, 10H, and the 210H. This well has had 0 out of 93 stages fracked. The 9.625 inch diameter annular pressure is 117 psi. The 13.375 inch diameter annular pressure is zero. Frack is currently shut down due to lighting strikes within 10 miles.
