-407112230 2018-1-9 0:00:0 DD No Violations 1.00
0407112230 2018-01-09 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 tripping back into the hole after a bit trip. 8.75 inch diameter hole is currently drilled to 6416 feet.
0652056697 2017-10-16 Casing String Witnessed No Violations On the day of my inspection I witnessed express energy cement the 20 inch diameter conductor casing. A 30 inch diameter casing was set to 40 feet and the backside was ready-mixed. The pad is on strip ground and they had to stabilize the unconsolidated material to reach total depth. A 26 inch diameter hole was augered to 120 feet. Express cemented the 20 inch diameter casing with 237 sacks of class A cement. Cement displaced with 39 barrels of water. Cement weighed 15.6 with mud scale and has a yield of 1.18. Additives include 2% cacl and .5#/sk celloflake. 50% excess ran in open hole from 40 to 120. 10 barrels of cement returned to surface.
0845215546 2018-01-02 Production Wells No Violations On the day of my inspection I witnessed Patterson 580 had just finished cementing their 13.375 inch diameter casing. The rig drilled a 17.5 inch diameter hole to a depth of 521 feet. Wyoming casing ran 514 feet of casing; j-55, 54.5 lbs. per foot. Casing centralized with 12 centralizers. Shoe track is 39 feet long. Previous casing cemented was 20 inch diameter conducter to 120 feet. O-tex was onsite with 525 sacks of class H cement and displaced the cement with 74 barrels of water. 50% excess ran in open hole. Cement crew says 19 barrels of cement returned to surface and floats held with 0.5 barrel back to the truck.
0906248909 2018-03-12 Preliminary Restoration No Violations On the day of my inspection I found the pad to include 5 wells. During my inspection the wells were having their wellheads built. Patterson 580 moved off of the pad last week and these wells have not been stimulated. The pad is stable and vegetated. Site meets requirements of ORC 1509.072A for preliminary restoration.

1503814719 2018-01-29 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 was drilling their 8.75 inch diameter hole at 6950 feet. They are drilling with oil base mud and are in the clinton. They got stuck in the hole and had to sidetrack last week. A cement plug and whipstock were set from 5,675 to 5,069 feet. Bottom hole assembly consisting of 4 drill collars, directional tool, mud motor, 2 crossover subs, stabilizer, and bit was left in the hole along with 7,935 feet of 5.875 inch diameter drill pipe.
1516129089 2018-01-16 Drill / Deepen / Reopen No Violations On the day of my inspection I witnessed Patterson 580 was drilling their 8.5 inch diameter lateral section at a depth of 13932.
