0354902402 2018-04-10 Preliminary Restoration No Violations On the day of my inspection I found this pad containing 8 properly labeled wells that are producing. This well has a plunger lift system wellhead. Pressure gauges read as follows: Surface 23 psi, Intermediate 208 psi, Production 1141 psi, and Tubing 0 psi. Casing hanger is plumbed into buried flow line, along with the intermediate annular. Production equipment includes fifteen 400 barrel steel tanks within their own steel containment, three compressors, three line heaters, eight sand separators, eight GPUs, a vapor recovery stack, and two combustors. One heater treater onsite. Site is stable and vegetated. Site meets preliminary restoration requirements of 1509.072A.
0422545769 2016-02-26 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. I found that all the wells have been drilled and night caps are placed on the wells bores. A gas sales line has been ran to the location but was not connected. No production equipment was present on the day of my inspection.
0689272687 2015-06-19 Casing String Witnessed No Violations upon arrival I found the crew running 13 3/8 inch casing to the depth of 508 feet. Schlumberger will do the cementing. Established circulation and circulated 17 bbls of good cement returns back to surface. Floats held and 1 /4 bbl of water returned to tank when releasing the pressure.
1434864360 2015-06-20 Plug / Plug Back No Violations I was contacted by Chesapeake Energy to advise me that they had lost tools in the hole and intended to plug back. They plan to whip stock around the lost hole. I was advised that because they had called cementers in preparation for the intermediate cement job that they are on site and would be ready to set the plug. The plan was to shoot off the drill string at 1569 ft. and trip out. Next go in the well with open ended drill pipe and set a 500 ft. cement plug. I arrived to find them just shooting off. They decided that the cement on site was not correct to set the whip plug so now they are waiting for another truck to return after loading the correct cement and think it will be 5 AM
1458724484 2015-06-13 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Express Energy Service drilled a 26 inch diameter hole to 120 ft. and set 120 ft. of 94 lb. casing. Express Energy Service cemented with 230 sacks of class A cement at 15.6 lb. and a yield of 1.18 Had good cement returns to surface. 14 bbl.
