0298896752 2015-03-10 Casing String Witnessed No Violations I was on location at 7:45am to witness the cementing of the 13.375 surface casing. Nomac Rig #73 drilled a 17.5 hole to a total depth of 543 and Wyoming Casing set 533 of surface casing in the hole. Baker-Hughes rigged up, tested the lines, and cemented the casing with 105 barrels of Class A cement slurry. They released the plug and displaced the plug to the float collar with 75.8 barrels of freshwater. The plug landed at 270psi and the pressure was increased and held at 877psi for 5 minutes. The pressure was released and 0.5 barrels flowed back to the truck. 25 barrels of cement circulated to surface. No violations. See cement ticket for more details on this job.
0474168944 2016-02-18 Preliminary Restoration No Violations I arrived on location accompanied by inspector Mark Moreland, were I witnessed the lease road gate was locked and the lease road in adequate condition. The pad is fenced and all man doors were locked. The permitted wells are producing off of natural reservoir pressure. RBDMS production records indicate that the wells are in producing order.
The production equipment on the pad consists of the following:

12 400 bbl. Production tanks 8 Producing wells 8 Sand separators

2 Horizontal heater-treaters 1 combustor 1 Low pressure separator tower/Vapor recovery system unit. 5 compressors

There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A.
0739099428 2015-03-16 Drill / Deepen / Reopen No Violations I was on location to check the drilling status of Nomac Rig #73. They are currently at a depth of 8,399 with the 8.5 drill bit and are tripping drill pipe out of the hole to change out the motor and bit in the curve section or they will miss the projected landing point of 8,662. The mud-up-point was 7,156 and the kick-off-point was 7,256. The location is in good condition and all drilling equipment is on containment. No violations.
1392710870 2015-03-11 Casing String Witnessed No Violations I was on location at 8:40pm to witness the cementing of the 9.625 intermediate casing. Nomac #73 drilled a 12.25hole to a depth of 2431 and Wyoming Casing ran 2421 of intermediate casing into the hole. Baker-Hughes rigged up, tested the lines, and pumped 10 barrels of water, 30 barrels of gel, and another 10 barrels of water spacer ahead of the slurry. They then mixed and pumped 186.2 barrels of cement dropped the plug and displaced the plug to the collar with 176.8 barrels of water. The plug landed at 1000psi and the pressure was increased and held at 1500psi for 5 minutes. The pressure was released and the floats held with 1 barrel flowing back to the truck. 40 barrels of cement circulated to surface. No violations. See cement ticket for more details on this job.
2113432835 2016-01-11 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a production tree. Production equipment for the well included a sand trap, a gas production unit and a line heater. Production equipment that was shared with permitted wells 34067212280000, 34067212290000, 34067212300000, 34067212310000, 34067212320000, 34067212330000, and 34067214310000 included a vapor recovery system, 5 gas compressors, a gas combustor and 12-400 bbl. tanks that where placed inside proper containment. The site passed preliminary restoration in accordance with ORC 1509.072 A.
