0460770857 2016-09-07 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road gate was locked and the lease road in adequate condition. The permitted well is producing off of natural reservoir pressure. RBDMS records indicate that the well is in producing order. The production tubing is being monitored using a Psi gauge placed at the top of the well head. At the time of my inspection the production tubing gauge read approximately 2200 psi. Fluids and gases could be transferring from the wellhead to the flow line. I use unable to capture the pressure readings of the surface casing and intermediate casing due to the fact it was not clear what position the valves were in. The gases coming from the tanks were being flared off.

Production equipment on the well pad consisted of the following. 4- 400 barrel production tanks 1- Condensate compressor station. 4- Gas Production Units with built in line heaters and sand separators 1- Flare stack. 5- Producing wells utilizing natural reservoir pressure. The site passed preliminary restoration in accordance with ORC 1509.072 A.

0532093962 2017-12-11 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 5 producing wells. Wells and production equipment are on different pads. The surface casing had 0 psi and the intermediate casing had 0 psi. Wellheads were labeled. Production equipment includes five GPUs, four 400 barrel steel tanks, a compressor, a heater treater, and a flare. Production records are current. Site meets prelimary restoration requirements of ORC 1509.072A.
1070275233 2015-08-25 Drill / Deepen / Reopen No Violations Upon arrival on locat5ion I found the nippling up the BOP. The are doing their test, and preparing to start drilling 12.75 inch hole.
1440291549 2015-08-22 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 13-3/8 inch diameter casing string. HWD Drilling Rig # 11 drilled a 17.5 inch hole to 536 ft. and set 529 ft. of 54.5 lb. casing. Schlumberger Cementing Service cemented with 520 sacks of class H cement at 15.6 lb. and a yield of 1.20 -- circulated 18 bbl. Of good cement returns to surface. Had problems with the cement pump truck and other equipment.
1441029365 2015-08-31 Casing String Witnessed No Violations I am on site this morning to witness the running and the cementing of the 9-5/8 inch diameter casing string. HWD # 11 drilled a 12-1/4 inch hole to 5046 ft. and are having problems with the casing and the mandrel which got hung up in the BOP stack. They are to be ready around 6 am. I returned at 6 AM and they are still running behind and will not be ready until 9 AM they will set 5036 ft. of 40 lb. casing. Schlumberger Cementing Service will cement with 259 bbl.- 760 sacks Of class H cement at 12 lb. and a yield of 1.91 lead cement and 108 bbl. -495 sacks of class A cement at 15.6 lb. and a yield of 1.22 tail cement and had good cement returns to surface.

2028500242 2015-08-30 Casing String Witnessed No Violations I was advised that they would be cementing between 8 to 10 PM and when I arrived I found that they are having trouble running the 9-5/8 inch diameter casing. There are some other equipment failures occurring as well and it appears that they will not be ready until morning to cement casing.
