0080218706 | 2015-01-07 | Drill / Deepen / Reopen | No Violations | I am on site today to check on the clean up status of the drilling operation. Nomac Drilling Rig # 74 is done drilling and will rig down and move to Tuscarawas Co Parker Pad. Check on clean up of the drilling cuttings and could not get closed enough to check entire site because of rig move. Will return after drilling rig has moved and check on site to make sure clean up is complete. |
1254161684 | 2016-02-18 | Preliminary Restoration | No Violations
Production equipment that is on site but not in use consists of. 2 400 bbl. production tanks 2 well bores 2 Sand Traps There was approximately 2 to 3 inches of snow on location at the time of my inspection. The site passed preliminary restoration in accordance with ORC 1509.072 A. | |
1405819387 | 2015-01-20 | Production Wells | No Violations | I am on site today to check on the clean up of the drill cuttings from outside of containment and on the old containment which was left around the well. The cuttings have been removed along with the old liner used for the drilling containment. It has been rolled up and the well area is clean. There is equipment entering the location today in preparation for the completion of the well. |
1418327291 | 2014-12-11 | Casing String Witnessed | No Violations | I am on site today to witness the running and the cementing of the 20 inch diameter casing string. Top Hole Drilling Rig # 1 drilled a 26 inch diameter hole to 120ft and set 120 ft. of 20 inch diameter casing. Express Energy Service cemented with 230 sacks of class A cement at 15.6 lb. and a yield of 1.18 and had 12 bbl. Of good cement returns. Express was late arriving to the job. |
1419874050 | 2014-12-29 | Drill / Deepen / Reopen | No Violations | I arrived today to conduct a rig inspection and found that the Nomac Drilling Rig # 74 is on site drilling. They set surface on Christmas and I did not witness the cement job. During my inspection I noticed that the liner looked to be leaking and collected samples with a quantab. The readings went to 750 PPM and up to 850 PPM. It also appears that drill cutting has been mixed off containment and may be part of the chloride problem. I met the Company Man Mike Vanston to go over the area that needs cleaned up. He is to bring in a Super Sucker Vacuum truck that will suck up the rock and stone along with the cuttings and fluid on site which is contaminated. The liner is to be removed, replaced and leak tested to make sure it holds fluid. |
1977147055 | 2014-12-30 | Drill / Deepen / Reopen | No Violations | I returned today to check on the clean up of the chlorides and drill cuttings outside the containment area. I found that the drill cuttings which spilled outside of containment have been cleaned up along with fluids. The corner of the containment has been repaired. Today I found that during the drilling operation while still drilling on air that the half round tanks had been unloaded by air pressure and had dumped the contents of the tank outside on both sides of the tank. There are drill cuttings and fluids dumped over that entire area. I met with the Company Man Mike Vanston again and advised that the cuttings while on containment needed to be cleaned up. The containment liner had holes in multiple locations the previous day which they made repairs on. Those are the ones which could be seen and there is no guarantees that the liner is leak proof. Mike advised me that there was vacuum trucks on the way to pick up the spill and to wash down the area. |
1999278743 | 2015-01-28 | Completion Testing | No Violations | On the date of inspection I was on site to check the current operations on the well pad. I was informed by Chesapeake employee, Brian Kinney, that the stimulation of the well had recently been completed. Chesapeake was removing equipment and frac tanks from the site and moving them to another location. |
