0065137403 2015-11-02 Drill / Deepen / Reopen No Violations Inspector Moreland and I are on site to pick up cement information from the company man John Mode. While on site I also conducted a solids control check. With the solids control check, I found everything clean and in containment. Nabors drilling rig No.36 is currently drilling the nudge section of the well bore with a 8 3/4 inch diameter hole on air.
0574822435 2016-09-07 Preliminary Restoration No Violations On the day of my inspection I witnessed the lease road gate was locked and the lease road in adequate condition. The permitted well is producing off of natural reservoir pressure. RBDMS records indicate that the well is in producing order. The production tubing is being monitored using a Psi gauge placed at the top of the well head. At the time of my inspection the production tubing gauge read approximately 3000 psi. Fluids and gases could be transferring from the wellhead to the flow line. I use unable to capture the pressure readings of the surface casing and intermediate casing due to the fact it was not clear what position the valves were in. The gases coming from the tanks were being flared off.

Production equipment on the well pad consisted of the following. 4- 400 barrel production tanks 1- Condensate compressor station. 4- Gas Production Units with built in line heaters and sand separators 1- Flare stack. 5- Producing wells utilizing natural reservoir pressure. The site passed preliminary restoration in accordance with ORC 1509.072 A.

1203115573 2015-10-26 Drill / Deepen / Reopen No Violations Inspector Moreland and I arrived on location to check on the drilling status of Nabors Rig No.36. Conducted an inspection of solids control, found everything clean and in order. There were no leaks or spills outside of containment. Met with the company man Roy Chaeaney, he informed us that they were picking up the BHA (Bottom Hole Assembly) and will spud the intermediate section within the hour.

Schlumberger Cementing Service cemented with 25 bbl. of gel ahead and 560 sacks of class H cement at 15.6 lb. and a yield of 1.20---circulated 10 bbls of good cement returns to surface.

1821660804 2017-12-11 Preliminary Restoration No Violations On the day of my inspection I found this pad to contain 5 producing wells. Wells and production equipment are on different pads. The surface casing had 0 psi and the intermediate casing had 600 psi. Wellheads were labeled. Production equipment includes five GPUs, four 400 barrel steel tanks, a compressor, a heater treater, and a flare. Production records are current. Site meets prelimary restoration requirements of ORC 1509.072A.
