0027572412 2018-03-15 Drill / Deepen / Reopen No Violations On the day of my inspection I found this pad to contain 1 properly labeled producing well and four conductors/cellars that are not labeled. Production equipment includes one GPU, a vapor recovery tower, combustor, compressor, and four 500 barrel steel tanks. Steel tanks are in metal containment and there is a berm around pad. There is no drilling equipment onsite. Outslopes of pad are stable and vegetated. Pad is fenced and locked. Per ORC 1509.072A this permit is not eligible for a preliminary restoration until it is drilled to total depth.
0050391486 2018-12-26 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Wireline just came out of this well on stage 34. Plug is set at 13121 with bottom perforation at 13111 and top perforation at 12991. Intermediate annular pressure was 412 psi.
0570582466 2018-12-20 Completion Testing No Violations On the day of my inspection I was accompanied by Geologist Tammy Richards. A halliburton frac crew is on location. Stage 10/65 had plug set at 16721, with bottom perf at 16711 and top perf at 16591. Equipment is down for maintenance and no frac is ongoing. Surface annular pressure is 400 psi and intermediate annular is 0 psi.
0621594573 2019-01-03 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew performing a final pressure test on kill plugs in the 1H well. This well had two kill plugs set in it today at 7945 and 7830. Plugs passed a negitive and a positive pressure test. Halliburton will be moving off of location starting today. Surface and intermediate annular pressures are 100 and 250 respectively.

Current hole depth is 8776 feet with an 8.75 inch diameter bit. Hole will be 8.5 inch diameter from here to total depth. Identified myself to company man Larry Hart.

1539260131 2018-10-11 Casing String Witnessed No Violations On the day of my inspection I witnessed the second stage of the 9.625 inch diameter cement job. Patterson 795 drilled a 12.25 inch diameter hole to a depth of 7356 feet. Wyoming ran 7348 feet of 9.625 inch diameter casing, 40#/ft, p-110. Shoe track is 90 feet long. DV tool set at 3492. Weatherford inflatible packer set at 3294. Packer set at 2650 psi. On the first stage, Schlumberger pumped 40 barrels of 11 ppg mudpush, 140 barrels of class A lead cement, and 120.4 barrels of class A tail cement. Lead cement weighed 12 ppg and yield of 1.79. Tail cement weighed 15.6 ppg and yield of 1.19. Cement displaced with 550 barrels of oil base mud. 20% excess cement ran in open hole. Set packer and opened DV tool to circulate well. 30 barrels of mudpush returned to surface. Started second stage. Schlumberger again pumped 40 barrels of 11 ppg mudpush, 245 barrels of class A cement weighing 15.6, yield 1.19. cement displaced with 265 barrels of oil based mud. 35% excess cement ran in open hole.

1715515688 2018-12-04 Completion Testing No Violations On the day of my inspection I witnessed a Halliburton frac crew on location. Surface annular pressure is 0 and intermediate annular pressure is 250. They have not started stimulation on this well yet.
1896684341 2019-01-29 Completion Testing No Violations On the day of my inspection I witnessed CWS rig 7009 onsite. This well has had frac plugs drilled out and 3803 feet of 2.875 inch diameter tubing ran. Surface annular pressure is 150 and intermediate annular pressure was 400.
2117096258 2019-05-08 Preliminary Restoration No Violations On the day of my inspection I am visiting pad at the request of Adam Schroeder on behalf of Mr. Humpreys. Mr. Humpreys was concerned with no production on the 9H well which was due to shutting it in to drill the 1H, 3H, 5H, and 7H wells. Email was sent to Adam Schroeder informing him the well is back in production. This well is currently producing off of natural reservoir pressure. Production equipment includes five GPUs, five sand seperators, two line heaters, one combustor, three compressors, two vapor recovery towers, and eight 500 barrel steel tanks, within their own containment. The 13.375x9.625 annular pressure was 170 psi and the 9.625x5.5 annular pressure is 327 psi. Pad is stable and vegetated. Site meets requirements of 1509.072 (A) for preliminary restoration.
