0737244850 2016-03-08 Production Wells No Violations On the day of my inspection accompanied by Inspector Mark Moreland following up on a spill that had occurred the previous night, I witnessed the spill was remediated. Prior to our arrival on site Mark placed a call to Rick Trippel to confirm that the use of the PID (Photoionization Detector) was an adequate method to scan the effected spill area. When we arrived on location Brad Burwell stated that the spill clean-up was completed at 3 am. Brad then took us around the spill area, equipped with a PID, to insure us that the spill was properly excessed. With the PID meter I witnessed there was one reading of 4 ppm of volatiles. The PID meter could not detect any other readings on site.
0830362999 2016-12-02 Production Wells No Violations I am on site today to perform a routine status check of the well bore and the well location. On the day of my inspection I witnessed the lease load was in good repair, however slight erosion was noticed at the beginning of the lease road and the gates for the location were locked. After inspecting the well bore I discovered no high annular pressures and that the well was in producing order. With the inspection I did not discover any indications of pollution or contamination. While on site I spoke to well tender Nick Kuthy who informed me that the wells have been preforming as projected.

1268131918 2016-01-29 Production Wells No Violations On the day of my inspection accompanied by Inspector Mark Moreland following up on a noise complaint issued by a nearby landowner Tom Zedman; I witnessed the loud noise that Tom had noticed was remediated. After assuring the issue was resolved, Mark then made a call to Tom. Tom stated that he was satisfied with the efforts Gulfport had made in resolving the loud noise. The complaint can now be considered resolved.
1411735479 2014-09-26 Drill / Deepen / Reopen No Violations Rocky Mountain Drilling Service is drilling a 32 diameter hole today and will be ready to cement casing later today.
1412774706 2014-10-08 Casing String Witnessed No Violations I am on site today to witness the running and the cementing of the 18-5/8 inch diameter casing string. Patterson Drilling Rig # 256 drilled a 24 inch diameter hole to 325 ft. and set 325 ft. of 87.5lb casing. Stingray Cementing Service cemented with 510 sacks of class A cement at 15.6 lb. and a yield of 1.20 and had 10 bbl. Of cement returns. Had no circulation until the last 20 bbl. of displacement.
1413466029 2014-10-15 Drill / Deepen / Reopen No Violations Patterson Drilling Rig # 257 is at 6003 ft. today drilling on air to set the deep intermediate casing string. They should be ready to set casing Sat.
1425316267 2015-03-02 Production Wells No Violations I am on site today to check on the Service Rig status. There is a snubbing unit over the well today. It is set up on containment and a check of the general area did not reveal any leaks or problems. There are crews on site installing the production tanks and lines.
1454423713 2016-01-28 Production Wells No Violations I arrived in location accompanied by Inspector Mark Moreland in regards to a complaint that Mark had received early in the day. Tom Zedman placed a call to Mark in concerns of a loud noise that was coming from the well pad. Tom stated that noise could be heard clearly from his residence and it was becoming an annoyance to him and his neighbors. At the time of arrival the noise was present. The source of the noise appeared to come from the vapor recovery flare stack. It was easily understood why Tom had an issue with the noise. After conforming the source of the noise, Mark called Brad Burwell(Gulfport representative). Mark informed Brad of the issues occurring from the well pad. Brad then stated that he would send a production supervisor out to remedy the issue. After approximately 15 minutes a production supervisor arrived on site and fixed the issue.

1526365837 2016-04-28 Production Wells No Violations On the day of my inspection, I witnessed no signs of pollution or contamination on site and all wells were in producing order. I also discovered Gulfport was in the process of removing the compressors off location. When I spoke to Dave Gump (Production Supervisor), he informed me that Gulfport was planning on trying to flare off all low pressure gas that could not make it into the sales line. Proper identification was in place on the well heads and at the entry point going into the lease road. No violations noted at the time of my inspection.
1576663092 2015-04-29 Production Wells No Violations I am on site today to check this pad for chlorides and found that the levels are almost below detection. The well has all of the equipment in place to operate and appears to be in production.
2080081073 2014-09-26 Casing String Witnessed No Violations On the day of my inspection I found Rocky Mountain Drilling installing 26 inch casing in a 32 inch hole, to a depth of 110 feet. String Ray will be cementing the casing back to surface. Established circulation, and circulated 7 bbls of good cement returns to surface.
2089450267 2016-01-07 Preliminary Restoration No Violations On the day of my inspection, I witnessed the well was equipped with a Cactus Wellhead and a production tree. Production equipment for the well included a sand trap and a gas production unit. Production equipment that was shared with permitted wells 34067213840000 and 34067213820000 included a flare scrubber, vapor recovery system, flare stack and 8-400bbl production tanks that were placed inside proper containment dikes. The site passed preliminary restoration in accordance with ORC 1509.072 A.
